retinoblastoma (pediatric) hakkında
Bilinmesi gerekenler
The commonest sign is a white glow in the pupil in photographs, called leukocoria. Others are a squint, a red painful eye or poor vision. Diagnosis uses examination under anaesthetic, ultrasound and MRI, and genetic testing for the RB1 gene. Children with an inherited form need regular eye screening, and siblings may be screened too.
Treatment aims to cure while saving vision where possible. Options include chemotherapy given into an artery of the eye or into the vein, laser, freezing treatment, plaque radiotherapy and injections into the eye. Large tumours may need removal of the eye (enucleation), with chemotherapy afterwards if there are high-risk features. Families travel for specialist ocular oncology centres with intra-arterial chemotherapy.
Yolculuk öncesi tetkikler
Önceden tamamlanmış olması gereken testler
- Examination under anaesthesia report from a specialist, with fundus drawings or retinal photographs mapping each tumour and any vitreous or subretinal seeding.
- MRI of the brain and orbits, to look for optic nerve involvement and to exclude pineal tumours.
- Ultrasound of the eyes and details of the child's age at diagnosis, eye laterality and family history.
- Genetic test results for the RB1 gene, either in blood or tumour, which affect risk to the other eye and to relatives.
- Any treatments already given, such as chemotherapy, laser, freezing treatment or eye surgery, with dates.
İkinci görüş
Ne zaman ikinci görüş alınmalı
- Your child has been advised to have an eye removed and you want to confirm that saving the eye is not possible.
- The cancer is in both eyes, or the disease is advanced, which requires a centre with experience of specialised treatment.
- Treatment has not worked or the tumours have returned.
- Radiotherapy is being suggested, which carries long-term risks in children with the hereditary form, and you want alternatives reviewed.
Doktorunuzla konuşurken
Sorulacak sorular
- What group or stage is my child's retinoblastoma, and is there any sign it has spread beyond the eye?
- Can the eye be saved with chemotherapy given into an artery or the eye, rather than removal?
- If the eye must be removed, what will the artificial eye and follow-up involve?
- Does my child have the hereditary form, and should siblings and parents be tested?
- What are the risks of radiotherapy and what treatments avoid it?
- How long will examinations under anaesthetic continue, and what is the risk of second cancers?
Prognoz
Gerçekçi prognoz
With early diagnosis and specialist treatment, survival for retinoblastoma confined to the eye is very high, well over 90 percent in well-resourced settings. Outlook worsens if the cancer has spread along the optic nerve or outside the eye. Eye salvage depends on tumour size and stage. Children with the hereditary form need lifelong monitoring for other cancers.

Will my child lose the eye?
Not always. Many eyes can be saved with chemotherapy and local treatments such as laser or freezing, especially in early disease. Eye removal is needed for advanced tumours to protect life.
Is retinoblastoma hereditary?
Some children carry an RB1 gene change that can be passed on, particularly when both eyes are affected. Genetic testing guides screening of siblings and future children.
What follow-up is needed?
Frequent eye examinations, often under anaesthesia, continue for years. Children with the hereditary form need long-term awareness of other cancers.
Can my child lead a normal life?
Most children treated successfully do, including those with one eye or an artificial eye. Vision support may be needed if both eyes were affected.
Clinical context
Working up retinoblastoma (pediatric) properly
A condition like retinoblastoma (pediatric) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a onkoloji specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or onkoloji specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
