Pelvic Organ Prolapse

Pelvic organ prolapse is when the bladder, womb or bowel bulges into or out of the vagina because the supporting muscles and ligaments have weakened. It is most common after childbirth and menopause.

pelvic organ prolapse hakkında

Bilinmesi gerekenler

Women report a dragging sensation or lump in the vagina, urinary leakage or incomplete emptying, constipation, and discomfort during sex. Diagnosis is by pelvic examination, graded by how far the organs descend. Prolapse is not dangerous, and symptoms often worsen with standing or straining and through the day.

First-line care is pelvic floor exercises with a physiotherapist, weight loss, treating constipation and cough, and a vaginal ring pessary. Vaginal oestrogen may help dryness. Surgery is for those who want a lasting solution and includes vaginal repair, uterus-preserving operations such as Manchester repair, and sacrocolpopexy done laparoscopically or robotically. Treatment abroad is sometimes sought for minimally invasive surgery.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • A gynaecology or urogynaecology examination report giving the type and grade of prolapse (for example POP-Q staging) of each compartment.
  • Urine tests, bladder diary and urodynamic results if you have leakage, urgency or difficulty emptying.
  • A record of symptoms: bulge, dragging, urinary and bowel problems, and effect on sexual function.
  • Details of past pregnancies and births, hysterectomy or earlier prolapse or incontinence surgery, including any mesh used.
  • Cervical screening status, and pelvic ultrasound if there is bleeding or a suspected womb problem.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • Surgery is advised for a mild prolapse with few symptoms before pelvic floor physiotherapy or a pessary has been tried.
  • Mesh is proposed and you have not been told the specific risks and the alternatives.
  • Prolapse has returned after previous surgery, which often needs a more specialised assessment.
  • You are planning a pregnancy, or you want to keep your womb, and no one has discussed womb-preserving options.

Doktorunuzla konuşurken

Sorulacak sorular

  1. Which organ is prolapsing and how severe is it?
  2. Would pelvic floor exercises or a pessary be reasonable first steps?
  3. What operation do you recommend, and does it use mesh or my own tissue?
  4. Can the womb be kept and is this wise in my case?
  5. What is the chance of the prolapse returning?
  6. Could surgery affect bladder, bowel or sexual function?

Prognoz

Gerçekçi prognoz

Prolapse is not dangerous in itself, and many women manage well with pelvic floor exercises, lifestyle changes or a pessary. Surgery relieves the bulge in most women, but a proportion experience recurrence over the years, particularly with heavy lifting, chronic cough, constipation or obesity. Outcome depends on the type and severity of prolapse, tissue quality, age, type of repair and the number of previous repairs.

  2. Önce yerel bir jinekoloji uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Does prolapse always need surgery?

No. Mild prolapse often only needs exercises or a pessary, and surgery is for symptoms that bother you.

Can it come back after surgery?

Yes, a minority of women have recurrence, and risk is higher with heavy lifting, obesity and chronic cough or constipation.

Is it safe to have another baby?

Pregnancy and delivery can worsen prolapse. If you plan children, surgery is usually delayed.

What about mesh?

Mesh is used in some abdominal repairs with good results, but vaginal mesh has risks, so discuss them fully.

Clinical context

Working up pelvic organ prolapse properly

A condition like pelvic organ prolapse is rarely well-served by jumping straight to treatment shopping. The sequence below is what a jinekoloji specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or jinekoloji specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
