Azoospermia (Non-Obstructive)

Non-obstructive azoospermia means there is no sperm in the semen because the testes are producing very little or none. It is found in a small proportion of infertile men.

azoospermia (non-obstructive) hakkında

Bilinmesi gerekenler

There are usually no symptoms apart from failure to conceive. Diagnosis involves two semen analyses, hormone tests such as FSH and testosterone, genetic tests including karyotype and Y-chromosome microdeletions, and examination of the testes. The cause may be genetic, hormonal, or follow infection, undescended testes or cancer treatment.

Hormonal therapy helps when the cause is a pituitary problem. In most other cases, sperm may still be found within the testis, and microsurgical testicular sperm extraction (micro-TESE) searches for it under an operating microscope. Any sperm found is used for ICSI with the woman's eggs. Success depends largely on the cause. Patients often travel abroad to specialist male-fertility units.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • At least two semen analysis reports, ideally with a centrifuged sample check.
  • Hormone tests including FSH, LH, testosterone and prolactin.
  • A genetic report: karyotype and Y-chromosome microdeletion testing, which are usually requested.
  • A scrotal ultrasound report and any testicular biopsy or surgery records.
  • History of undescended testes, mumps, chemotherapy, radiotherapy or medicines affecting sperm.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You have been told sperm retrieval is impossible, or no testicular sperm retrieval attempt has been made by a micro-TESE specialist.
  • A Y-chromosome microdeletion or abnormal karyotype has been found and you need counselling.
  • A previous retrieval found no sperm and a second attempt is being discussed.
  • Your hormone levels suggest a treatable endocrine cause that has not been addressed.

Doktorunuzla konuşurken

Sorulacak sorular

  1. What is the likely cause of my azoospermia, and what do my genetics show?
  2. Is hormone treatment useful before attempting retrieval?
  3. How likely is sperm retrieval with micro-TESE in my case?
  4. What happens if no sperm is found, and what are the alternatives?
  5. Are there any genetic risks for children conceived with this sperm?
  6. How are retrieved sperm frozen and used with ICSI?

Prognoz

Gerçekçi prognoz

The chance of finding sperm depends on the cause: it is higher in some conditions, such as a history of cryptorchidism, and lower with Sertoli-cell-only pattern, Klinefelter-type findings differ, and certain AZFa/AZFb deletions. In many men microsurgical retrieval finds sperm, which can then achieve pregnancy through ICSI. Results depend on the underlying diagnosis, female partner age and the laboratory.

Does azoospermia mean I can never father a child?

No. Some men with non-obstructive azoospermia have small areas of sperm production in the testis. Surgical retrieval followed by ICSI can lead to biological fatherhood, though success is not guaranteed.

What is micro-TESE?

It is an operation in which the surgeon uses a microscope to find and remove sperm-producing tubules from the testis. It reduces tissue removed and improves the chance of finding sperm.

Is hormone treatment helpful?

In some men with low hormone levels from pituitary problems, treatment can restore sperm. In others, hormone therapy is used before retrieval; it depends on the cause.

Can the condition be passed to a son?

Some genetic causes, such as Y-chromosome microdeletions, can be passed on to sons conceived by ICSI. Genetic counselling is recommended.

Clinical context

Working up azoospermia (non-obstructive) properly

A condition like azoospermia (non-obstructive) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a fertilite ve tüp bebek specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or fertilite ve tüp bebek specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
