hepatocellular carcinoma (hcc) hakkında
Bilinmesi gerekenler
Early tumours cause no symptoms and are found by ultrasound surveillance in people with cirrhosis. Later signs include weight loss, abdominal pain, swelling and jaundice. Diagnosis often relies on typical CT or MRI appearances with a raised alpha-fetoprotein level, and sometimes biopsy. Treatment choice depends on tumour size, number and liver function.
Small tumours can be cured by surgical resection (hepatectomy), radiofrequency ablation or liver transplantation. For larger or multiple tumours, transarterial chemoembolisation (TACE) and Y-90 radioembolisation (SIRT) deliver treatment through the liver's blood supply. Advanced disease is treated with systemic drugs including immunotherapy. Treating the underlying liver disease is important. Patients may seek centres offering liver transplant or interventional radiology abroad.
Yolculuk öncesi tetkikler
Önceden tamamlanmış olması gereken testler
- Multiphase CT or MRI of the liver showing tumour size, number, location and vascular involvement.
- Liver function tests, platelet count, INR and bilirubin, with Child-Pugh and MELD score or ALBI grade.
- AFP tumour marker results over time, and hepatitis B and C virus status and viral load.
- Biopsy report if one was done; HCC is sometimes diagnosed on imaging alone in cirrhosis.
- Records of previous treatments such as ablation, TACE or resection, plus endoscopy for varices.
İkinci görüş
Ne zaman ikinci görüş alınmalı
- You are told the tumour is inoperable but have not been assessed for transplant, ablation or embolisation.
- Treatment is advised without clear staging, such as BCLC stage, or assessment of liver reserve.
- A single small tumour is found and you want to compare resection, ablation and transplantation.
- The cancer has returned after treatment, or systemic therapy is being considered.
Doktorunuzla konuşurken
Sorulacak sorular
- What stage is my cancer, and how well is my liver working?
- Am I suitable for resection, ablation, transplant or embolisation?
- What amount of liver can safely be removed?
- What is the chance of recurrence, and how will it be monitored?
- Should my hepatitis be treated, and could that help?
- If it cannot be cured, what treatment can control it?
Prognoz
Gerçekçi prognoz
Outlook depends on tumour stage and underlying liver function. Small single tumours treated by resection, ablation or transplant can be cured, with a substantial proportion alive at five years. Recurrence is common because the damaged liver remains at risk, so regular surveillance scans are essential. Larger or multifocal disease is treated with embolisation, radiotherapy or systemic drugs to control it. Treating the underlying hepatitis or fatty liver improves the outlook.

Can liver cancer be cured?
Small tumours found early can be cured with resection, ablation or transplant. Because the damaged liver can develop new tumours, regular surveillance continues.
Who needs a liver transplant?
People with small tumours within accepted size limits and cirrhosis, where removal is unsafe, may be considered. Transplant treats both cancer and the diseased liver.
What is TACE?
Transarterial chemoembolisation delivers chemotherapy to the tumour through its artery and blocks its blood supply. It is used for tumours that cannot be removed or as a bridge to transplant.
Clinical context
Working up hepatocellular carcinoma (hcc) properly
A condition like hepatocellular carcinoma (hcc) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a onkoloji specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or onkoloji specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
