Constrictive Pericarditis

Constrictive pericarditis is a stiff, thickened sac around the heart that stops the heart filling properly. It can follow infection, radiotherapy, cardiac surgery or tuberculosis.

constrictive pericarditis hakkında

Bilinmesi gerekenler

Symptoms include breathlessness, tiredness, swollen legs, a swollen abdomen from fluid and raised neck veins. Echocardiography, CT or MRI, and sometimes cardiac catheterisation, show the thickened pericardium and the abnormal filling pattern, and help tell it from restrictive heart muscle disease. It tends to progress slowly.

Water tablets help with fluid overload, and the cause, such as tuberculosis, is treated. Where inflammation is still active, anti-inflammatory medicines may produce some improvement. The definitive treatment is surgical removal of the pericardium, called pericardiectomy, performed by an experienced cardiac surgeon. Treatment abroad is considered because of the specialist nature of the operation.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • Echocardiogram with respiratory variation of flow, and cardiac CT or MRI showing pericardial thickening or calcification.
  • Cardiac catheterisation report with pressure measurements, if done, to separate constriction from restrictive heart disease.
  • Records of the likely cause, such as past tuberculosis, previous heart surgery, radiotherapy or viral pericarditis.
  • Blood tests: liver function, kidney function, albumin and BNP, and tuberculosis tests where relevant.
  • A list of diuretics and other medicines, with doses and response.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • Your diagnosis was based on echo alone and constriction has not been separated from restrictive heart disease.
  • Surgery has been advised, but you have heavy calcification or liver damage and want an experienced centre's assessment.
  • Your symptoms are controlled with water tablets and you are unsure whether to proceed to surgery.

Doktorunuzla konuşurken

Sorulacak sorular

  1. How certain is the diagnosis of constriction, and what cause is likely?
  2. Is anti-tuberculosis treatment needed before surgery?
  3. What portion of the pericardium will be removed, and how risky is the operation for me?
  4. How soon will swelling and breathlessness improve afterwards?
  5. What follow-up will I need to check heart function?

Prognoz

Gerçekçi prognoz

Pericardiectomy relieves symptoms in most patients, though improvement can be gradual over weeks to months. Surgery carries meaningful risk, higher with advanced disease, calcification, poor heart or liver function, or radiation as the cause. Outlook is best when surgery is done before severe heart muscle and liver damage set in. If a reversible cause such as tuberculosis is treated early, some cases improve without surgery.

  2. Önce yerel bir kalp cerrahisi uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Is surgery the only treatment?

Water tablets ease swelling but do not cure constriction. Surgical removal of the pericardium is the definitive treatment, though some inflammatory or early cases improve with medicines.

Could tuberculosis be the cause?

In many countries, yes. If tuberculosis is suspected, anti-tuberculosis treatment is usually given, and surgery is timed after your doctor has judged the response.

Will I recover completely after surgery?

Most people feel much better, but recovery may take weeks to months, and those with long-standing disease, calcification or liver damage may have lasting symptoms.

Clinical context

Working up constrictive pericarditis properly

A condition like constrictive pericarditis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a kalp cerrahisi specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or kalp cerrahisi specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
