anal fissure (chronic) hakkında

Bilinmesi gerekenler

Typical symptoms are burning or cutting pain with bowel movements, spasm that can last hours, and small amounts of bright red blood on the paper. A doctor usually makes the diagnosis from the history and a gentle look at the area. Chronic fissures often show a skin tag and exposed muscle fibres, and they persist because the tight sphincter reduces blood flow to the tear.

First-line care is softer stools with fibre and fluids, plus ointments that relax the sphincter, such as glyceryl trinitrate or diltiazem. Botulinum toxin injection is an option when ointments fail. Lateral internal sphincterotomy, a small cut in the sphincter muscle, heals most fissures and is considered when other treatments do not work. Treatment abroad is rarely necessary, but is sometimes chosen for planned surgery.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • A clinical examination report by a colorectal surgeon noting the position of the fissure and whether a sentinel pile or fibres are visible.
  • A list of treatments already tried, with dates, such as stool softeners, nitrate or calcium-channel blocker ointments.
  • Details of bowel habit, including constipation, diarrhoea and any bleeding.
  • Colonoscopy or sigmoidoscopy results if there is bleeding, a fissure away from the midline, or age over 45 to 50.
  • Any history of inflammatory bowel disease, previous anal surgery or difficult childbirth, along with your medicine list.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • Your fissure is off the midline or multiple, which can point to Crohn's disease or another condition.
  • You have tried ointments and fibre for two to three months and still have pain, and surgery has been suggested.
  • You are worried about continence after sphincter surgery, particularly after childbirth or earlier anal operations.

Doktorunuzla konuşurken

Sorulacak sorular

  1. Is this a simple chronic fissure, or could another condition be causing it?
  2. Which non-surgical options remain, and for how long should I use them?
  3. What is the risk of altered bowel control after lateral sphincterotomy?
  4. Is botulinum toxin an option for me instead of surgery?
  5. How can I stop the fissure coming back?

Prognoz

Gerçekçi prognoz

Most chronic fissures heal with sphincterotomy, with healing rates commonly above 90 per cent, and pain usually eases within days. A small risk of minor gas or stool leakage exists, and it is higher in women after childbirth and in older patients. Ointments heal a smaller share, and recurrence is more likely if constipation continues.

  2. Önce yerel bir gi̇s cerrahisi uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Do I always need surgery?

No. Many fissures heal with stool softening, fibre and prescribed ointments. Surgery or botulinum toxin is considered when healing has not occurred after a trial of treatment.

Will the fissure come back?

It can, especially if constipation or hard stools persist. Fibre, fluids and prompt treatment of constipation lower the risk.

Is a fissure serious or linked to cancer?

Most are benign, but a fissure away from the midline, one that does not heal, or one with other bowel symptoms should be examined for conditions such as Crohn's disease.

How long is recovery after sphincterotomy?

Pain often eases within days and most people return to work in a few days to a week. The wound heals over several weeks.

Clinical context

Working up anal fissure (chronic) properly

A condition like anal fissure (chronic) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a gi̇s cerrahisi specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or gi̇s cerrahisi specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
