cancer-related fertility preservation hakkında

Bilinmesi gerekenler

Chemotherapy, pelvic radiotherapy and some surgeries can reduce or end fertility, depending on the drugs, doses, age and sex. Doctors should raise the issue before treatment starts, and a fertility specialist assesses ovarian reserve or sperm quality. The aim is to act quickly without delaying cancer care. Hormone-sensitive cancers need careful planning of any ovarian stimulation.

Men can bank sperm before treatment begins. Women can freeze eggs or embryos after a short course of hormone stimulation, typically about two weeks, and vitrification gives reliable survival after thawing. Ovarian tissue freezing is possible when there is no time to stimulate, and ovarian suppression or shielding may protect function in some cases. Frozen material can be used years later, and long-term storage rules vary by country.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • A letter from your oncologist giving the diagnosis, the planned treatment and how long treatment can safely be delayed.
  • For women, anti-Müllerian hormone, antral follicle count and a pelvic ultrasound report.
  • For men, a recent semen analysis and infectious disease screening results (HIV, hepatitis B and C).
  • Information about hormone-sensitive cancer, since some stimulation protocols are adapted to protect against hormone exposure.
  • Details of any previous fertility treatment, blood clotting issues and current medicines.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You have been told there is no time for fertility preservation, and you want to know if a quicker protocol is possible.
  • Your cancer is hormone-sensitive and you are unsure how egg stimulation can be made safe.
  • You are a prepubertal child and options such as tissue freezing are being explored.
  • Your oncologist and fertility specialist have given conflicting advice about timing.

Doktorunuzla konuşurken

Sorulacak sorular

  1. How long can my cancer treatment safely be delayed?
  2. How likely is my planned treatment to affect my fertility?
  3. Which option suits me best: eggs, embryos, sperm, or tissue?
  4. Is the stimulation protocol safe for my type of cancer?
  5. How long can samples be stored, and what are the legal rules?
  6. Will I be able to use them after treatment, and when?

Prognoz

Gerçekçi prognoz

Fertility preservation does not treat the cancer, and it does not guarantee a future pregnancy. Success depends mostly on age at freezing and the number of eggs, embryos or sperm stored. Sperm freezing and embryo or egg vitrification are well established, with frozen eggs and embryos giving pregnancy rates close to fresh ones in younger women. Ovarian tissue freezing is used when there is no time for stimulation. Starting preservation must not unsafely delay cancer care.

  2. Önce yerel bir onkoloji uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Will this delay my cancer treatment?

Egg stimulation takes about two weeks, and sperm banking can be done in days. Your oncologist decides whether this delay is acceptable.

Is it safe if my cancer is hormone-sensitive?

Special protocols that limit oestrogen exposure are available and are discussed with your oncologist.

How long can eggs or sperm be stored?

Frozen samples can be stored for many years, subject to local law.

Can I freeze if I'm a child or teenager?

Options depend on puberty; tissue freezing may be offered in specialised centres.

Clinical context

Working up cancer-related fertility preservation properly

A condition like cancer-related fertility preservation is rarely well-served by jumping straight to treatment shopping. The sequence below is what a onkoloji specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or onkoloji specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
