Anal Fistula

An anal fistula is an abnormal tunnel between the inside of the anal canal and the skin near the anus. It usually follows an anal abscess that did not heal fully.

anal fistula hakkında

Bilinmesi gerekenler

Typical features are a persistent discharge, pain, swelling, recurrent abscesses and skin irritation around the opening. Diagnosis is by examination under anaesthetic when needed, and MRI maps the tunnel and its relation to the sphincter muscles. Fistulas rarely heal on their own, and untreated ones can cause repeated infection.

Treatment is surgical, because antibiotics do not cure it. The choice depends on the amount of sphincter muscle the tract crosses. Simple fistulas can be laid open, while complex ones use sphincter-saving methods such as LIFT, video-assisted treatment (VAAFT), advancement flaps or seton drains. The aim is to close the tunnel while protecting bowel control. Fistulas in Crohn's disease need combined medical care.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • MRI pelvis or endoanal ultrasound report mapping the tract and its relation to the sphincter muscles.
  • Operative notes from any previous abscess drainage or fistula surgery, including whether a seton was placed.
  • Colonoscopy and Crohn's disease assessment, since Crohn's can cause complex fistulas.
  • Description of symptoms such as discharge, pain, swelling and any problems with bowel control.
  • Details of diabetes, steroids or immune suppression, which affect healing.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You are advised to have a cutting operation (fistulotomy) on a fistula that crosses a lot of sphincter muscle, which risks incontinence.
  • You have Crohn's disease or have had several failed operations.
  • Symptoms have returned after surgery and you want to know about LIFT, VAAFT or flaps.
  • No MRI has been done to map the tract before surgery.

Doktorunuzla konuşurken

Sorulacak sorular

  1. Is my fistula simple or complex, and which part of the sphincter does it involve?
  2. Which operation gives the best chance of healing with the least risk to continence?
  3. Do I need a seton first, and for how long?
  4. Could Crohn's disease be behind this, and should I be tested?
  5. What is the chance the fistula comes back?
  6. How long is the recovery, and what wound care will be needed?

Prognoz

Gerçekçi prognoz

Healing rates depend on the type of fistula. Simple low fistulas usually heal after surgery, whereas complex or high fistulas need staged or sphincter-sparing procedures such as LIFT and may recur. The risk of weakened bowel control rises with the amount of muscle involved. Crohn's disease, previous surgery and the location of the tract affect the results.

  2. Önce yerel bir gi̇s cerrahisi uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Can a fistula heal by itself?

Rarely. Because the tunnel is lined with tissue, it usually persists and causes recurrent abscesses, so surgical treatment is normally needed.

Will surgery affect my bowel control?

Risk depends on the amount of muscle involved. Sphincter-sparing techniques such as LIFT reduce this risk compared with cutting through muscle.

Can it come back after surgery?

Yes, in a proportion of patients, especially with complex fistulas or Crohn's disease. Your surgeon will explain the risk for your type.

Clinical context

Working up anal fistula properly

A condition like anal fistula is rarely well-served by jumping straight to treatment shopping. The sequence below is what a gi̇s cerrahisi specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or gi̇s cerrahisi specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
