Adenomyosis

Adenomyosis is a condition in which the womb lining grows into the muscular wall of the womb. It commonly causes heavy, painful periods and a tender, enlarged womb, mostly in women in their 30s to 50s.

adenomyosis hakkında

Bilinmesi gerekenler

Symptoms include heavy or prolonged bleeding, severe period pain, pelvic pressure and sometimes pain during sex. Transvaginal ultrasound or MRI shows a thickened, uneven muscle wall, and the diagnosis is often confirmed only after hysterectomy. It often eases after the menopause, and it can coexist with fibroids or endometriosis.

First-line treatment is medicine: anti-inflammatory painkillers, tranexamic acid, the hormonal coil or other hormonal treatments. If these fail, options include uterine artery embolisation, endometrial ablation for selected cases, or hysterectomy, which is the only definitive cure. Choice depends on symptoms and fertility plans. Treatment abroad is mainly considered for embolisation or minimally invasive hysterectomy.

Yolculuk öncesi tetkikler

Önceden tamamlanmış olması gereken testler

  • Pelvic ultrasound or MRI report describing thickening of the uterine wall, and whether the disease is diffuse or focal.
  • Blood count and ferritin, since heavy bleeding commonly causes iron deficiency anaemia.
  • Records of hormonal treatments tried, such as the levonorgestrel coil, progestogens or tranexamic acid.
  • Cervical screening result and, where relevant, endometrial sampling or hysteroscopy findings.
  • Your fertility plans and obstetric history, which decide whether the womb should be preserved.

İkinci görüş

Ne zaman ikinci görüş alınmalı

  • You are told a hysterectomy is the only choice although you wish to keep fertility or the womb.
  • Symptoms persist after hormonal treatment and you want to compare embolisation, ablation and surgery.
  • Scans are unclear and fibroids, adenomyosis or both may be present.
  • You are anaemic from heavy bleeding and uncertain how urgent treatment is.

Doktorunuzla konuşurken

Sorulacak sorular

  1. How certain is the diagnosis, and is the adenomyosis diffuse or in one area?
  2. Which treatments protect my fertility, and which do not?
  3. How effective is uterine artery embolisation for adenomyosis compared with hysterectomy?
  4. Could endometriosis or fibroids also be contributing to my pain?
  5. Do I need treatment for anaemia first?
  6. What happens to symptoms at menopause?

Prognoz

Gerçekçi prognoz

Adenomyosis is benign and symptoms usually ease after menopause. Hormonal treatment can control bleeding and pain for many women, and embolisation helps a good proportion, although some symptoms return over time. Hysterectomy is the only definitive cure. Fertility is possible but may be reduced, and results depend on how extensive the disease is and on your age.

  2. Önce yerel bir jinekoloji uzmanına gidin

Evde yüz yüze klinik muayene — tek bir konsültasyon bile — düşük maliyetli ve destinasyon planını önemli ölçüde iyileştirir. Yanınıza alın: yazılı klinik değerlendirme, önerilen prosedür, cerrahi risk sınıfı ve varsa kontrendikasyonlar.

  3. Ücretsiz ikinci görüş için dosyayı bize gönderin

Tıbbi panelimizden iki cerrah değerlendirmeyi bağımsız olarak inceler. Ameliyat mı yapacaklarını, izleme mi alacaklarını, yoksa yönlendireceklerini mi söylerler — seyahate karşı tavsiyeleri bile olsa.

Will adenomyosis go away?

Symptoms usually improve after menopause, but until then it persists. Hormonal treatment and other procedures reduce symptoms rather than remove the condition, except hysterectomy.

Can I get pregnant with adenomyosis?

Yes, though fertility may be lower and risks such as miscarriage or preterm birth may be higher. A fertility specialist can advise on timing and options.

Do I need a hysterectomy?

Not necessarily. Many women are managed with hormonal treatment or procedures such as embolisation. Hysterectomy is a definitive option if you have completed your family and other treatments have failed.

Clinical context

Working up adenomyosis properly

A condition like adenomyosis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a jinekoloji specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or jinekoloji specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
