О процедуре Тиреоидэктомия

Under general anaesthesia the surgeon makes a small horizontal incision low in the neck and carefully frees the gland while protecting the voice nerves and the parathyroid glands beside it. A lobectomy removes one side, while a total thyroidectomy removes both. Some centres offer endoscopic or robotic approaches that avoid a neck scar.

The operation relieves pressure symptoms and gives a definite diagnosis for nodules. After a total thyroidectomy you will need thyroid hormone tablets for life, and after a partial removal you may or may not. Risks include temporary or permanent low calcium and voice change, which are lower with experienced thyroid surgeons.

Plan about two days in hospital and a short stay nearby for the wound check. Bring ultrasound, biopsy and thyroid blood test results. At home you will need blood tests to adjust your hormone dose, usually six to eight weeks after surgery, so arrange a doctor who can follow this up.

Обычно подходит, если

  • A large goitre causing pressure, difficulty swallowing or breathing.
  • A nodule that is suspicious or cancerous on biopsy.
  • Overactive thyroid not controlled by tablets or radioiodine, or where those are unsuitable.
  • A goitre extending behind the breastbone.

Возможно, стоит подождать, если

  • Uncontrolled overactive thyroid, which needs stabilising before surgery.
  • Heart or lung disease that makes general anaesthesia risky without further assessment.
  • A small, clearly benign nodule without symptoms, where monitoring may be enough.
  • Bleeding disorders or blood thinners not yet managed by your doctor.
Как подготовиться
  1. Bring ultrasound reports, fine needle aspiration results, thyroid blood tests and any scans of the neck or chest.
  2. Ask for a vocal cord check by laryngoscopy before surgery, particularly if your voice is already hoarse or you have had neck surgery.
  3. Review blood thinners, anti-thyroid drugs and other medicines with your doctor; do not change them yourself.
  4. If you have an overactive thyroid, your doctor may first stabilise your hormone levels.
  5. Plan to stay close to the hospital until calcium levels are stable and you know your thyroid hormone dose.
Вопросы, которые стоит задать хирургу
  1. Do I need a hemithyroidectomy or total thyroidectomy, and why?
  2. Do you use nerve monitoring, and will my vocal cords be checked before and after?
  3. How do you protect my parathyroid glands, and how often do patients at your centre develop low calcium?
  4. Will I need lifelong thyroid hormone tablets, and who adjusts the dose?
  5. Where will the scar be and how will it be cared for?
  6. How many thyroid operations do you perform each year?
На что обратить внимание при выборе клиники
  • No pre-operative vocal cord check or no mention of nerve injury risk.
  • Recommending total thyroidectomy for a small benign nodule without explaining why.
  • No plan to check calcium after total removal.
  • Surgery by someone who rarely performs thyroid operations.
