Urethral Stricture

A urethral stricture is a scarred narrowing of the urethra, the tube that carries urine out of the bladder. It mostly affects men, often after injury, catheter use, infection or earlier urethral surgery.

О urethral stricture

The usual signs are a weak or spraying stream, straining, a feeling of incomplete emptying and repeated urinary infections. Diagnosis is by flow-rate testing, a urethral X-ray with contrast and a flexible camera examination. Untreated strictures tend to narrow further and can lead to retention or bladder damage.

Dilatation or a small cut through the scar (urethrotomy) gives short-term relief, but the stricture often returns. For lasting results, particularly with longer or recurrent strictures, open reconstructive surgery called urethroplasty is the standard, using the patient's own tissue. Because the operation is specialised, some patients travel abroad to a reconstructive urologist.

  • A retrograde urethrogram and voiding cystourethrogram, with images, showing the location and length of the stricture.
  • Uroflowmetry results and a post-void residual urine measurement.
  • A urine culture, and kidney function blood tests if urine flow has been poor for a long time.
  • A full record of prior treatments, such as dilations, urethrotomy or catheter use, with dates and how long each helped.
  • Details of the likely cause, such as injury, infection, catheterisation or previous surgery.

  • You have had two or more dilations or urethrotomies and the narrowing keeps returning.
  • You have been advised to continue repeated dilation without any discussion of urethroplasty.
  • The stricture is long, complex or follows radiation or pelvic fracture injury.
  • You are unable to pass urine and have a catheter in place, and the long-term plan is unclear.

  1. How long and where is my stricture, and what is the likely cause?
  2. What is the chance that a further urethrotomy or dilation will work for me?
  3. Would urethroplasty be better, and which technique do you propose?
  4. What are the risks to erections, ejaculation and continence?
  5. How long will I need a catheter afterwards?
  6. How will recurrence be monitored and what is the follow-up plan?

Strictures often return after dilation or urethrotomy, especially after the first repeat, so these are mainly short-term measures. Urethroplasty has the highest long-term success, and most men remain free of recurrence for years, with results best for short strictures. Success depends on stricture length, location, cause and surgical expertise. Follow-up with flow tests is important because recurrence, when it happens, is usually within the first couple of years.

Why does my stricture keep coming back after treatment?

Dilation and urethrotomy cut or stretch the scar, but the scar often re-forms. This is why repeat procedures are less and less likely to work and why urethroplasty is considered after a second recurrence.

What is urethroplasty?

It is an operation that removes the scarred segment or widens it with a graft or flap, rebuilding the urethra. It has the highest long-term success rate for urethral stricture.

Can I do anything to prevent recurrence?

Treating the cause, avoiding unnecessary catheters, and following your surgeon's instructions help. Some surgeons advise self-dilatation after urethrotomy. Report a weakening stream early.

Can I fly with a stricture?

Usually yes, but if you are at risk of sudden retention of urine, you should have a plan for emergency care. Discuss this with your urologist before travelling.

Clinical context

Working up urethral stricture properly

A condition like urethral stricture is rarely well-served by jumping straight to treatment shopping. The sequence below is what a урология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or урология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
