Syringomyelia
Syringomyelia is a fluid-filled cavity, called a syrinx, within the spinal cord. It is often linked to a Chiari malformation, past spinal injury or scarring.

A syrinx can enlarge slowly over years, causing neck or arm pain, loss of temperature and pain sensation in the hands, weakness, stiffness in the legs and, in some, scoliosis. MRI of the brain and spine confirms the diagnosis and looks for the underlying cause. Some small syrinxes never change and cause no problem, so progress is judged by symptoms and repeat scans.
If the syrinx is small and symptoms are mild or stable, regular MRI and neurological review are enough. Surgery is considered for progressive weakness or pain. When a Chiari malformation is the cause, decompression of the skull base restores fluid flow and often shrinks the syrinx; otherwise a shunt may drain it. Specialist neurosurgical opinion is the main reason people look at treatment abroad.

Syringomyelia often progresses slowly, and some people with a small syrinx and few symptoms stay stable for years. Surgery to correct the cause, such as decompression for a Chiari malformation, usually halts progression and reduces the cavity in most patients. Pain, stiffness and existing nerve damage may improve only partly, and recovery is best when treated before significant weakness develops. Long-term follow-up with MRI is needed.
