О gallstones (symptomatic)
The typical attack is a steady pain under the right ribs or in the upper middle abdomen, lasting from 30 minutes to a few hours, often after a fatty meal. Fever, jaundice or severe pain point to infection or a duct stone and need urgent care. Ultrasound shows the stones, and blood tests and MRCP assess the bile ducts. Attacks usually recur.
Painkillers and low-fat meals help in the short term, but stones do not dissolve reliably. The standard treatment is laparoscopic cholecystectomy, keyhole removal of the gallbladder, usually done as a day case or with one night in hospital. Stones in the bile duct are cleared by ERCP. Most people live normally without a gallbladder. Treatment abroad may be considered for planned keyhole surgery, but not during an acute attack.
- Abdominal ultrasound report and images, showing the stones, gallbladder wall thickness and bile duct width.
- Liver function tests and pancreatic enzymes, particularly if you have had jaundice or severe pain.
- A record of pain attacks: how often, how long and how they are triggered, and any hospital admissions.
- MRCP or endoscopic ultrasound results if stones in the bile duct are suspected.
- Details of other conditions and medicines, especially blood thinners, diabetes and any previous abdominal surgery.
- You have been told stones need removal but have never had symptoms, as stones found by chance often need no treatment.
- You have had jaundice, fever or pancreatitis, because the bile duct may also need clearing.
- Your pain persists after the gallbladder is removed.
- Your symptoms are vague, such as bloating or indigestion, and a link to gallstones is uncertain.
- Are my symptoms really caused by gallstones, or could something else explain them?
- Are any stones in the bile duct, and does that need treatment first?
- Can the operation be done by keyhole surgery, and what is the chance of switching to open?
- What are the risks, such as bile duct injury, and how often do they occur?
- How long until I can eat normally and return to work?
- What happens if I decide not to have surgery?
Removing the gallbladder cures gallstone attacks in most people with typical symptoms and is one of the most common and safest abdominal operations. Recovery after keyhole surgery is usually quick. Without treatment, attacks often recur and complications such as infection or pancreatitis can occur. Some people notice looser stools afterwards, but life without a gallbladder is generally normal.

Do I have to remove my gallbladder?
If gallstones cause repeated attacks or complications, removal is the usual advice because attacks tend to recur. Stones that cause no symptoms generally do not need surgery.
Can gallstones come back after surgery?
Stones cannot form in a gallbladder that has been removed. Rarely, stones in the bile duct can develop or be missed, which can cause similar pain.
How long is recovery after keyhole gallbladder removal?
Most people go home the same day or the next, return to light activity within days and to normal work in about one to two weeks, depending on the job and any complications.
Clinical context
Working up gallstones (symptomatic) properly
A condition like gallstones (symptomatic) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a хирургия жкт specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or хирургия жкт specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
