Retinal detachment is a sight-threatening emergency in which the retina peels away from the back of the eye. It is most common in short-sighted people, after cataract surgery or eye injury, and in older adults.

Warning signs are sudden flashes of light, a shower of new floaters and a shadow or curtain spreading across the vision, sometimes with blurred central sight. Diagnosis is by a dilated retinal examination, with ultrasound if the view is blocked. Without prompt repair the retina can die and sight is lost, so same-day assessment is advised.

Surgery is almost always needed. Small tears may be sealed with laser or freezing treatment before detachment. Detachments are repaired by vitrectomy, scleral buckle or pneumatic retinopexy, depending on the type and position of the break, often with gas or silicone oil in the eye. The earlier the macula is reattached the better the vision, so travelling abroad is rarely advisable unless it can be done quickly.

  • The eye surgeon's examination report describing which parts of the retina are detached, whether the macula is involved and where the tears are.
  • Dilated retinal photographs or wide-field imaging, plus OCT of the macula, if available.
  • Your visual acuity measurements and eye pressure readings.
  • A history of eye surgery such as cataract operation, high myopia, eye injury, laser treatment and any previous retinal problems in either eye.
  • A list of blood-thinning medicines and any flying restrictions, because gas bubbles placed in the eye make air travel unsafe.

  • You have new flashes, floaters or a shadow in your vision. This needs urgent examination, not waiting for a second opinion.
  • Your retina has detached repeatedly after surgery, or scarring called proliferative vitreoretinopathy has developed.
  • You have been advised on a surgical method and want to understand why buckle, vitrectomy or retinopexy was chosen.
  • The detachment is in your only seeing eye, or the other eye shows lattice degeneration or tears.

  1. Is my macula still attached, and how does that affect how quickly surgery should happen?
  2. Which operation do you recommend and why, and what are the alternatives?
  3. Will I have a gas bubble or silicone oil, and what position and travel restrictions apply?
  4. What are the chances the retina reattaches with one operation, and what could cause failure?
  5. What proportion of my vision is likely to return, and will cataract develop later?
  6. What should I watch for in the other eye?

Most detachments can be reattached, often with a single operation, but the visual result depends on how long the macula was detached and how good the vision was beforehand. Early repair gives the best chance of good vision. Some patients need further surgery, and cataract commonly develops after vitrectomy. Delay risks permanent loss of sight.

Is retinal detachment an emergency?

Yes. Sudden flashes, new floaters or a shadow across vision need examination within hours to a day. Early treatment gives the best chance of saving vision.

Can I fly after retinal surgery?

Not if a gas bubble has been placed in the eye, because pressure changes can cause dangerous pressure rises. Gas may last two to eight weeks. Silicone oil is safe for flying.

Can it happen in the other eye?

Yes, there is a higher risk in the second eye, particularly with high myopia or lattice degeneration. Learn the warning signs and have regular dilated examinations.

Will my vision go back to normal?

It depends on whether the macula was detached and for how long. Many people regain useful vision, though some blurring or distortion can remain.

Clinical context

Working up отслойка сетчатки properly

A condition like отслойка сетчатки is rarely well-served by jumping straight to treatment shopping. The sequence below is what a офтальмология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or офтальмология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
