О anovulation (refractory)

Typical signs are very irregular or missing periods and no conception after months of trying. Assessment includes hormone blood tests, an ultrasound scan of the ovaries and checks for thyroid disease, raised prolactin, body weight and a semen analysis. The label 'refractory' is used when letrozole or clomifene, sometimes with metformin, have not produced ovulation.

Treatment usually starts with weight management where relevant, then ovulation-induction tablets, then injectable gonadotrophins or laparoscopic ovarian drilling. If ovulation still fails, or other fertility factors exist, IVF with ICSI is the next step, because eggs are collected directly. Women often look abroad when repeated cycles have failed and want a different clinic's protocol.

  • Hormone results such as FSH, LH, oestradiol, AMH, prolactin, thyroid function and, where relevant, androgens, with the cycle day on which each was taken.
  • Pelvic ultrasound reports, ideally with an antral follicle count and ovarian appearance, and any follicle-tracking scans from previous treatment cycles.
  • A written record of every ovulation-induction drug tried, with doses, number of cycles and how the ovaries responded.
  • Tubal assessment such as a hysterosalpingogram, and a semen analysis for the male partner, because these decide which fertility treatment is appropriate.
  • Results for blood glucose or HbA1c and body weight trends, since insulin resistance and weight often influence ovulation.

  • You have had several cycles of letrozole or clomifene without ovulating or conceiving, and have been told the only option is a more intensive treatment.
  • You have been advised to go straight to IVF or ICSI without a clear explanation of why ovulation induction or other options are not suitable.
  • Your hormone results suggest premature ovarian insufficiency or very low ovarian reserve, which changes the advice significantly.
  • You overreacted to stimulation drugs, with many follicles or ovarian hyperstimulation, and want a safer stimulation plan.

  1. What is causing my anovulation, for example polycystic ovary syndrome, a hypothalamic cause or low ovarian reserve, and how was that confirmed?
  2. Which ovulation-induction options have I not yet tried, and is there a reason to move on to IVF now?
  3. Why is ICSI being suggested for me, and would standard IVF be enough if my partner's semen analysis is normal?
  4. What is the risk of ovarian hyperstimulation with my pattern of response, and how would it be prevented?
  5. How many treatment cycles should I realistically plan for, and when would you advise reviewing the plan?
  6. Would weight, insulin resistance or other health issues need treating first to improve my chances?

Most women with anovulation can be helped to ovulate, and many conceive once the right treatment is found. Outcomes depend mainly on age, ovarian reserve, the underlying cause and any other fertility factors. Polycystic ovary syndrome generally responds better than ovarian insufficiency. Success per cycle falls with age, so several cycles may be needed.

Do I always need IVF if ovulation drugs fail?

Not always. Depending on the cause, options may include adjusting drug doses, adding other medicines, treating weight or insulin resistance, or ovarian drilling. IVF is considered when these fail or when other fertility factors such as blocked tubes are present.

Can anovulation come back after I conceive?

Yes. Ovulation problems usually reflect an underlying condition such as polycystic ovary syndrome, so you may need treatment again for a later pregnancy. Some women ovulate more regularly after pregnancy, but this cannot be relied on.

Is anovulation the same as early menopause?

No. Anovulation means eggs are not released regularly, often with normal or high egg reserve. Premature ovarian insufficiency means the ovaries are running out of eggs. Blood tests such as FSH and AMH help tell them apart.

Clinical context

Working up anovulation (refractory) properly

A condition like anovulation (refractory) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a фертильность и эко specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or фертильность и эко specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
