О plantar fasciitis (refractory)

The typical symptom is sharp pain under the heel with the first steps in the morning or after resting, easing as you warm up. The diagnosis is clinical, based on where the foot is tender. X-ray or ultrasound may be used to rule out other causes, such as a stress fracture or nerve entrapment. The condition often settles within a year, but a minority have persistent pain.

First-line care is non-surgical: calf and plantar fascia stretching, supportive footwear or insoles, load management and sometimes a night splint. Shockwave therapy and a corticosteroid injection may be offered when pain persists, though injections carry a small risk of rupture. Surgery to release part of the fascia is rarely needed and reserved for pain lasting over 6 to 12 months despite proper conservative treatment.

  • Notes on treatments tried so far, such as stretching, orthoses, night splints, injections and shockwave, with durations.
  • Foot X-ray, and ultrasound or MRI showing the thickness of the plantar fascia and any tear.
  • Your body weight, activity level and footwear details, which strongly influence recovery.
  • Nerve symptoms history, such as burning or tingling, in case tarsal tunnel syndrome or nerve entrapment is the real cause.
  • Details and dates of any steroid injections, as repeated injections can weaken the fascia.

  • Your pain is not clearly under the heel or is worse at night, which may mean nerve, bone stress or other causes.
  • You have had repeated steroid injections and are now being offered surgery.
  • Surgery is proposed before at least six months of well-structured non-operative care.
  • You are told you need Achilles tendon surgery, which is rarely the usual treatment for this condition.

  1. Is my diagnosis definitely plantar fasciitis, and have other causes of heel pain been excluded?
  2. Which non-surgical options have I not yet tried properly, such as shockwave, loading exercises or orthoses?
  3. What does surgery involve, and what are its risks, including arch weakness?
  4. How long would recovery take before I can walk or work normally?
  5. Should I limit weight-bearing exercise while this settles?
  6. What if I keep managing it conservatively?

Plantar fasciitis usually settles, and most people improve within a year with stretching, load management and supportive footwear. A minority have symptoms for much longer. Surgery is a last resort and helps many, but not all, and can alter foot mechanics. Body weight, long periods on the feet and tight calf muscles slow recovery.

How long does it last?

Most cases settle within about a year, and many much sooner. A minority have symptoms for longer, which is why a further option may be considered.

Is a heel spur the cause?

Not usually. Many people with heel spurs have no pain, and many people with pain have no spur. The pain mainly comes from irritation of the fascia.

Will surgery fix it?

Surgery helps many people who have failed several months of good non-surgical care, but not all. It carries its own risks, so it is usually the last step.

Can I keep exercising?

Often yes, with modified activity. Low-impact exercise such as swimming or cycling is usually better tolerated than running while symptoms are active.

Clinical context

Working up plantar fasciitis (refractory) properly

A condition like plantar fasciitis (refractory) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ортопедия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ортопедия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
