О pituitary adenoma

Symptoms depend on the type. Larger tumours can cause headaches and loss of peripheral vision by pressing on the optic nerves, while hormone-producing ones can cause conditions such as acromegaly, Cushing disease, or abnormal breast milk production and missed periods. Diagnosis combines blood hormone tests, a pituitary MRI and an eye field test. Many small tumours grow slowly or not at all.

Treatment depends on the tumour type. A prolactinoma is usually treated first with medicines such as cabergoline. Other functioning or large tumours are typically removed through the nose by endoscopic transsphenoidal surgery, with radiotherapy or radiosurgery for any remaining tumour, and hormone replacement if needed. Small tumours causing no problems may just be monitored. Patients may look abroad for a high-volume pituitary surgical team.

  • Pituitary MRI with contrast, ideally a dedicated pituitary protocol, supplied as images on disc as well as the report.
  • Full pituitary hormone profile including prolactin, IGF-1, morning cortisol, thyroid hormones and sex hormones.
  • Formal visual field testing and visual acuity report if the tumour is near the optic chiasm.
  • Any dynamic tests already done, such as dexamethasone suppression or glucose tolerance for growth hormone.
  • A list of medicines you take, including cabergoline or hormone replacement, with doses and dates.

  • Your tumour is a prolactinoma and surgery is proposed before a trial of medicine such as cabergoline.
  • You have visual loss and nobody has measured your visual fields formally.
  • Hormone levels do not match the scan, for example a small tumour with very high hormones.
  • Your tumour is large or invades nearby structures and you are told it cannot be fully removed.

  1. Is my tumour secreting a hormone, and which one, and does that change whether I need surgery?
  2. Is my vision at risk, and how often should visual fields be checked?
  3. What are the chances of removing the whole tumour, given its size and position?
  4. Which pituitary hormones may need replacing after surgery, and is that permanent?
  5. How long will I be followed up with scans and blood tests?
  6. What happens if I watch a small tumour and do nothing?

Most pituitary adenomas are benign and slow growing. Prolactinomas usually respond to medicine, and surgery can cure many other secreting tumours, especially smaller ones. Results depend on tumour size, invasion of surrounding tissue and the surgeon's experience. Some tumours regrow, and some patients need lifelong hormone replacement or long-term monitoring.

Is a pituitary tumour cancer?

Almost all are benign. They cause problems by pressing on nearby structures such as the optic nerves, or by producing too much of a hormone.

Will I need surgery?

Not always. Small tumours without symptoms can be watched, and prolactinomas often shrink on medicine. Surgery is usually advised for tumours pressing on vision or secreting hormones that medicine cannot control.

Will my vision recover?

Vision often improves after the pressure is relieved, especially if loss is recent and mild. Long-standing severe loss may only partly recover.

Will I need hormone tablets afterwards?

Some patients do, if the normal pituitary is affected by the tumour or surgery. Hormone levels are rechecked after surgery to decide, and some replacement is temporary.

Clinical context

Working up pituitary adenoma properly

A condition like pituitary adenoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
