О pheochromocytoma

Typical symptoms come in episodes: pounding headache, sweating, palpitations, tremor and anxiety, alongside high blood pressure that may be hard to control. Diagnosis rests on blood or 24-hour urine tests for metanephrines, followed by CT or MRI of the adrenal glands and sometimes a specialised nuclear scan. Genetic testing is often offered because a meaningful share of cases are inherited. Most tumours are benign and grow slowly.

Treatment is surgical removal of the affected adrenal gland, usually by keyhole surgery. Before the operation, alpha-blocking medicines are taken for one to two weeks to control blood pressure and prevent dangerous surges during surgery. Lifelong yearly blood or urine checks follow. Patients sometimes travel abroad for this surgery, which is best done in a centre with experienced endocrine surgeons and anaesthetists.

  • Plasma free metanephrines or 24-hour urinary fractionated metanephrines and catecholamines, with the laboratory reference ranges.
  • CT or MRI of the abdomen and pelvis showing the size and exact location of the tumour.
  • Functional imaging such as MIBG or DOTATATE PET if the tumour is large, multiple or possibly outside the adrenal gland.
  • Blood pressure diary and ECG or echocardiogram, since the hormones can strain the heart.
  • Any genetic test results and family history of thyroid, parathyroid or adrenal tumours, because many cases are inherited.

  • Your hormone tests are borderline or were taken while on medicines that can distort the results, so the diagnosis is uncertain.
  • The tumour is large, on both sides, or outside the adrenal gland, which changes the surgical plan.
  • You are young or have relatives with similar tumours and nobody has discussed genetic testing.
  • Surgery has been offered without a planned period of blood pressure preparation beforehand.

  1. Do my hormone results and scans confirm pheochromocytoma, and could the tumour be outside the adrenal gland?
  2. How will my blood pressure be prepared before surgery, and for how long?
  3. Is keyhole surgery suitable for my tumour size, or is open surgery safer?
  4. Should I have genetic testing, and should my relatives be screened?
  5. What follow-up tests will I need after surgery, and for how many years?
  6. What risks do I run if the tumour is left in place?

Most pheochromocytomas are benign and surgery cures the high blood pressure in the majority of patients. Outcomes depend on proper preparation before surgery, tumour size, and whether it is inherited or malignant. A small proportion recur, so lifelong yearly biochemical follow-up is usually advised, especially with a genetic cause.

Is surgery always needed?

Surgery is the standard treatment because the tumour can cause dangerous blood pressure surges and rarely turns malignant. Medicines are used beforehand to control blood pressure and heart rate, not as a long-term alternative. Your team will decide on timing.

Can it come back?

A small proportion recur or appear elsewhere, more often with inherited forms, large tumours or tumours outside the adrenal gland. This is why yearly blood or urine hormone tests are usually advised for many years.

Is it safe to fly before surgery?

Many people can, once blood pressure is controlled with medicines. Flying with uncontrolled surges is a risk, so ask your doctor to confirm you are stable and carry your medicines and reports in hand luggage.

Is it inherited?

About a third of cases are linked to a gene variant, and younger patients are more likely to have one. Genetic testing is usually offered, and if positive, relatives can be screened.

Clinical context

Working up pheochromocytoma properly

A condition like pheochromocytoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a хирургия жкт specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or хирургия жкт specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
