О peritoneal carcinomatosis
Symptoms include abdominal swelling from fluid, bloating, pain, nausea, loss of appetite and weight loss, or bowel obstruction. Diagnosis is by CT or MRI, sometimes with laparoscopy and biopsy to find the extent of disease. It is often hard to see on scans, and the spread is a key factor in outlook.
Treatment depends on the original cancer and spread. Systemic chemotherapy is standard, and fluid drainage and palliative care relieve symptoms. For carefully selected patients with limited disease, cytoreductive surgery with heated intraperitoneal chemotherapy (HIPEC) removes visible tumour and treats the remainder. It is major surgery. Treatment abroad is sought for HIPEC at high-volume centres.
- Pathology reports for the original cancer and any peritoneal biopsy, including grade and molecular results such as MSI and RAS/BRAF status.
- CT scan of chest, abdomen and pelvis, and MRI or PET-CT if done, to show extent of disease.
- The peritoneal cancer index (PCI) score, if a surgeon or laparoscopy has calculated one.
- Treatment history: chemotherapy regimens, response, surgery notes and the dates of each.
- Blood tests including tumour markers, blood counts, kidney, liver and nutritional status, and a performance-status assessment.
- Your team has advised palliative care only and no peritoneal surface oncology centre has reviewed your scans.
- Surgery with HIPEC is offered and the extent of disease, tumour type and your fitness have not been discussed clearly.
- The cancer is from an appendix, pseudomyxoma or mesothelioma and a specialist unit has not been consulted.
- The disease has recurred after earlier HIPEC and further options are being decided.
- What is the original cancer and how extensive is the disease in the abdomen?
- Am I a suitable candidate for cytoreductive surgery and HIPEC?
- Is complete removal of visible disease likely?
- What are the risks and the recovery time for this major operation?
- What chemotherapy comes before or after surgery?
- What outcome can I realistically expect compared with chemotherapy alone?
Outlook depends heavily on the type of tumour, how extensive the spread is and whether surgery can remove all visible disease. Selected patients with limited disease, for example from appendiceal or colorectal cancer, can achieve long-term control or sometimes cure after complete cytoreduction with HIPEC. Widespread disease or aggressive tumours have a poorer outlook. The operation is major with significant risks, so fitness and careful selection by an experienced centre matter.

Is HIPEC suitable for everyone?
No. It is for carefully selected patients with limited disease, good general health and a tumour type where benefit is established.
What does HIPEC involve?
After surgeons remove visible tumour, warmed chemotherapy is circulated within the abdomen for a set time during the same operation.
How long is recovery?
Hospital stay is often one to two weeks or longer, with several months to regain strength.
Can the cancer come back?
Yes, recurrence is possible, so regular scans and follow-up are needed.
Clinical context
Working up peritoneal carcinomatosis properly
A condition like peritoneal carcinomatosis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
