Wilms tumour is a cancer of the kidney in young children, most often found between the ages of two and five. It is the most common kidney cancer in childhood.

Most children are otherwise well and a parent or doctor finds a painless lump or swelling in the abdomen. Some have blood in the urine, high blood pressure or fever. Diagnosis is by ultrasound, CT or MRI, and the tumour is staged and examined under the microscope after surgery.
Treatment combines surgery to remove the affected kidney with chemotherapy, and radiotherapy for more advanced stages, with the order depending on the protocol used. Treatment is standardised, and outcomes are very good for most children. Long-term follow-up checks the remaining kidney and heart. Treatment abroad is considered for second opinions or complex bilateral disease.

Wilms tumour is among the most curable childhood cancers, with overall survival around 90 per cent in well-resourced settings for favourable histology, and high for early stages. Outlook is poorer for anaplastic histology, advanced stage, bilateral disease or relapse. Outcome depends on stage, histology, response to chemotherapy and complete surgical removal. Survivors need follow-up for kidney function, blood pressure and effects of chemotherapy or radiotherapy.
