Pediatric Spina Bifida

Spina bifida is a birth defect in which the spine does not close fully, leaving the spinal cord or its coverings exposed or damaged. It most often affects the lower back.

О pediatric spina bifida

The condition ranges from a hidden form with few effects to myelomeningocele, with weakness or paralysis of the legs, bladder and bowel problems, and hydrocephalus (fluid build-up in the brain). It is often found on pregnancy scans, with confirmation by MRI. Folic acid before and in early pregnancy reduces the risk of neural tube defects.

The defect is closed surgically soon after birth or sometimes before birth. Hydrocephalus is treated with a shunt to drain fluid. Lifelong multidisciplinary care covers mobility aids, physiotherapy, bladder management and bowel care, and orthopaedic and urological surgery as needed. Treatment abroad is often considered for fetal surgery or specialist spina bifida clinics.

  • Prenatal scan reports and the closure operation notes, with lesion level (thoracic, lumbar, sacral) and whether it was closed before or after birth.
  • MRI or ultrasound of the brain showing hydrocephalus and Chiari malformation.
  • Shunt history if present: date, type, revisions and any infections.
  • Urological assessment: kidney ultrasound, bladder studies and current catheterisation plan.
  • Orthopaedic and neurological assessment of leg strength, hips, feet and spine, plus pressure sore and latex allergy history.

  • Your child has headache, vomiting, drowsiness or a swollen fontanelle, which suggest shunt blockage and need urgent review.
  • Repeated urine infections or falling kidney function are present and bladder studies have not been done.
  • A tethered cord is suspected because of new weakness, pain or spinal curve.
  • You are told no more can be done for mobility or continence without a multidisciplinary review.

  1. What level is the defect and what function can we expect?
  2. Does my child have hydrocephalus or Chiari malformation, and how will it be managed?
  3. What bladder and bowel programme do you recommend?
  4. What warning signs mean the shunt is blocked or infected?
  5. What mobility aids, therapy and orthopaedic care will help?
  6. Who coordinates long-term care as my child grows?

Spina bifida is lifelong but, with modern care, most children survive into adulthood and many live independently. Abilities vary widely with the level and severity of the lesion, the presence of hydrocephalus and shunt complications, and bladder, bowel and orthopaedic management. Learning needs are common in some children. Outcomes are best with coordinated multidisciplinary follow-up, protection of the kidneys, prevention of pressure sores, and prompt treatment of shunt problems.

Will my child walk?

It depends on the level of the lesion. Many children with lower lesions walk with or without aids, while higher lesions often need wheelchairs.

Will a shunt last forever?

Not always. Shunts can block or become infected and may need revisions, so warning signs must be known.

Can the nerve damage be reversed?

Closure prevents further damage but generally does not restore lost function.

Can we prevent it in future pregnancies?

Folic acid taken before conception and in early pregnancy substantially lowers the risk; speak to your doctor about the dose.

Clinical context

Working up pediatric spina bifida properly

A condition like pediatric spina bifida is rarely well-served by jumping straight to treatment shopping. The sequence below is what a неврология и нейрохирургия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or неврология и нейрохирургия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
