Patent ductus arteriosus (PDA) is a blood vessel linking the aorta and pulmonary artery that normally closes soon after birth but stays open. It is most common in premature babies and can also be found in older children.

A small PDA may cause no symptoms and is found when a doctor hears a continuous murmur. A larger one makes the lungs overfilled with blood, causing fast breathing, poor feeding, slow weight gain and sweating. Diagnosis is by echocardiography. In premature babies the vessel often closes on its own or with medicines, while in larger children it rarely closes spontaneously.

Medicines such as ibuprofen or paracetamol may close the duct in premature babies. In older infants and children, closure is advised when the duct is significant, using a device delivered through a catheter, or surgical ligation in small infants. Very small ducts may only need follow-up. Treatment abroad is usually considered for experienced paediatric cardiac teams.

  • Echocardiogram report giving the duct size, direction of blood flow, and the effect on the heart chambers and lung pressures.
  • Chest X-ray and ECG reports, usually requested to show heart size and lung blood flow.
  • A growth and symptom record: feeding, breathing effort, sweating, weight gain and any repeated chest infections.
  • For premature babies, the neonatal summary, including gestational age, any medicines given to close the duct, and breathing support needed.
  • Blood tests and any prior cardiology letters, plus oxygen saturation readings.

  • A small duct with no symptoms is being closed urgently, or a large duct with poor growth and breathlessness is being only watched.
  • You are told that closure is needed but have not been given the choice between catheter device closure and surgical ligation.
  • The echocardiogram suggests raised lung pressures, which changes whether closure is safe.
  • Your child has other heart defects and it is unclear whether the duct is helping their circulation.

  1. How large is the duct, and is it causing strain on the heart or lungs?
  2. Does it need closing, or could it close on its own or be watched?
  3. Is device closure through a catheter suitable, or is surgery safer for my child's size?
  4. What are the risks of the procedure, and how long will the hospital stay be?
  5. Will my child need antibiotics before dental work or any restrictions afterwards?
  6. What follow-up echocardiograms will be needed?

Outcomes after closing a PDA are very good. Catheter or surgical closure is successful in the great majority of children, and most go on to have normal heart function and activity. Small ducts may never cause problems. Left open, a large duct can lead to heart failure, repeated chest infections and, over years, raised lung pressures. Prognosis depends on duct size, the child's age and weight, prematurity, associated heart defects and whether lung pressures have already risen.

Does every PDA need closing?

No. Small ducts without symptoms are often just watched, and some in premature babies close by themselves. Larger ducts causing heart strain, breathlessness or poor growth are usually closed.

Is closure by catheter or surgery better?

It depends on the child's size and duct shape. Catheter device closure suits many children and avoids a chest incision, while surgery is used for very small infants or unusual anatomy.

Can my child fly before closure?

Often yes if the duct is small and the child is well, but a large duct with breathlessness or high lung pressures needs cardiology advice first.

Can the duct reopen?

This is uncommon after successful closure. A follow-up echocardiogram confirms the duct is fully closed.

Clinical context

Working up аденома гипофиза properly

A condition like аденома гипофиза is rarely well-served by jumping straight to treatment shopping. The sequence below is what a кардиохирургия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or кардиохирургия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
