О meniere's disease
Attacks last from twenty minutes to several hours and can cause nausea and vomiting. Diagnosis rests on the pattern of symptoms and a hearing test showing low-frequency loss, with other causes such as migraine or tumours excluded, sometimes by MRI. Hearing tends to decline slowly over the years, while the vertigo attacks often become less frequent with time.
Treatment begins with reducing salt intake, avoiding triggers, and medicines for attacks such as anti-sickness drugs and betahistine. Injections of steroid or gentamicin into the middle ear help many people whose vertigo persists. Surgery on the inner ear or balance nerve is reserved for severe cases that do not respond. Treatment abroad is seldom needed, other than for specialist neuro-otology opinions.
- Recent audiograms (pure-tone hearing tests); serial tests over time show whether hearing is fluctuating or declining.
- A diary of vertigo attacks, noting date, length, severity and any triggers such as salt, stress or caffeine.
- MRI of the internal auditory canals, which is usually requested to exclude a tumour on the hearing nerve.
- Balance test reports, if done, such as caloric or video head impulse testing.
- A list of medicines already tried, such as betahistine or diuretics, and your response to them.
- Attacks continue despite diet changes and medicines and an injection or surgical procedure is being suggested.
- You have had sudden hearing loss or fainting-type falls that could point to another cause.
- Only one ear is affected and you are offered a destructive procedure that would remove balance function in that ear.
- No MRI has been done and the diagnosis rests on symptoms alone.
- Do my hearing tests and attack pattern meet the criteria for definite Meniere's disease?
- Has a tumour or other ear or brain cause been excluded?
- What treatments reduce the number of attacks before we consider anything that risks hearing?
- If a procedure is advised, what is the chance it affects my hearing or balance in the other ear?
- What should I do during an attack, and is it safe for me to drive?
- Could the other ear become involved over time?
Meniere's disease is long-lasting but attacks often become less frequent over years, and most people achieve good control of vertigo with diet, medicines or ear injections. Hearing loss and tinnitus tend to persist or worsen slowly, and some people eventually develop involvement of both ears. Results depend on how early it is treated and which ear is affected.

Will I lose my hearing completely?
Total loss in the affected ear is uncommon, but hearing often declines gradually, and tinnitus can persist. Treatment aims to reduce attacks and protect hearing. Regular hearing tests show how your ears are changing.
What triggers Meniere's attacks?
Triggers differ, but many people notice links with high salt intake, caffeine, alcohol, stress, poor sleep or allergies. A diary of attacks helps you and your doctor identify yours.
Is it safe to fly with Meniere's disease?
Many people fly without problems. Attacks can occur at any time, so carry your medicines, avoid travelling during an active period if possible, and discuss it with your doctor. Pressure changes do not reliably trigger attacks.
Can Meniere's disease affect both ears?
Yes, a proportion of people eventually develop it in the second ear, often within some years. This is one reason doctors avoid destructive treatments in the first ear unless attacks are severe.
Clinical context
Working up meniere's disease properly
A condition like meniere's disease is rarely well-served by jumping straight to treatment shopping. The sequence below is what a лор / отоларингология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or лор / отоларингология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
