Lynch Syndrome (Surveillance)

Lynch syndrome is an inherited condition caused by a fault in a DNA mismatch repair gene, which greatly raises the lifetime risk of bowel, womb and some other cancers.

О lynch syndrome (surveillance)

The condition itself causes no symptoms. It is suspected when several relatives have bowel or womb cancer, especially at a young age, and confirmed by genetic testing. Tumour testing can also flag it. Relatives can then be tested. Carriers face a higher chance of cancers developing earlier than in the general population.

Management is surveillance: colonoscopy every one to two years from early adulthood, with checks for other cancers as advised, and aspirin considered in some people. Removal of polyps during colonoscopy lowers risk. If cancer develops in the colon, a colectomy is performed, sometimes extended to reduce the chance of another cancer. Risk-reducing womb and ovary surgery may be offered after childbearing. Treatment abroad is seldom needed except for specific surgery.

  • Genetic test report naming the gene and exact variant (MLH1, MSH2, MSH6, PMS2 or EPCAM), and whether the variant is pathogenic.
  • A full family tree showing relatives with colorectal, endometrial, ovarian, stomach or urinary cancers, with ages at diagnosis.
  • All colonoscopy reports with dates, the quality of bowel preparation and any polyps removed.
  • Pathology of any tumour, including microsatellite instability or mismatch repair protein staining.
  • Records of gynaecological surveillance and any aspirin use or previous cancer treatment.

  • Your genetic result is classified as uncertain, and you want specialist interpretation.
  • You have a confirmed variant but colonoscopy is advised less often than guidelines generally suggest.
  • Colectomy is proposed as prevention, and you want to compare it with surveillance.
  • You are a woman who has finished childbearing and want to discuss risk-reducing hysterectomy and ovary removal.

  1. Which gene is involved, and how does it change my risk of different cancers?
  2. How often should I have colonoscopy, and from what age?
  3. Do I need screening for womb, stomach or urinary cancers?
  4. Is there any role for aspirin or preventive surgery in my case?
  5. Should my brothers, sisters, parents and children be tested?
  6. If cancer develops, how does Lynch syndrome affect the choice of treatment?

Lynch syndrome is not itself a cancer, and outcomes for carriers are good when surveillance is regular, because pre-cancerous polyps are removed and cancers are found early. Lifetime risk varies by gene: for example, MLH1 and MSH2 carry higher colorectal cancer risk than PMS2. Cancers in carriers often respond well to immunotherapy. Surveillance usually continues for life.

Will I definitely get cancer?

No. Lynch syndrome raises lifetime risk substantially, but not everyone develops cancer. Regular surveillance finds and removes problems early.

How often is colonoscopy needed?

Commonly every one to two years from around the age of 20 to 25, depending on the gene and local guidelines. Your genetics team will set the schedule.

Do my relatives need testing?

Yes. Each first-degree relative has a 50 per cent chance of carrying the variant, and testing allows early surveillance in those who do.

Clinical context

Working up lynch syndrome (surveillance) properly

A condition like lynch syndrome (surveillance) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
