Lumbar Disc Herniation

A lumbar disc herniation occurs when the soft centre of a disc in the lower spine pushes through its outer wall and presses on a nearby nerve. It often causes sciatica and most often affects adults aged 30 to 50.

О lumbar disc herniation

Typical symptoms are low back pain with pain, numbness or tingling radiating down one leg, sometimes with weakness. Diagnosis is by examination and, if symptoms persist, MRI. Most herniations improve naturally over weeks to months. New leg weakness, or loss of bladder or bowel control or saddle numbness, needs emergency care.

Initial treatment is staying active, pain relief and physiotherapy, with nerve root injections sometimes used. Surgery is considered if severe leg pain continues beyond about six to twelve weeks or if weakness develops. Lumbar microdiscectomy removes the part of the disc pressing on the nerve through a small incision, and endoscopic spine surgery uses an even smaller approach. Treatment abroad is often considered for planned microdiscectomy.

  • MRI of the lumbar spine showing the level and side of the herniation, and the nerve root affected.
  • A neurological examination record documenting leg power, reflexes, sensation and straight-leg raise findings.
  • Details of treatments tried: physiotherapy, anti-inflammatory medicines, nerve root injections, and for how many weeks.
  • A note of any bladder, bowel or saddle numbness symptoms, which are emergencies, and any progressive weakness.
  • A list of medicines including blood thinners, and your general health and weight.

  • Surgery has been advised within the first few weeks, without a trial of conservative treatment and no weakness.
  • You have had a discectomy and the pain has come back, and you want to know if repeat surgery or fusion is appropriate.
  • Your MRI findings do not match your symptoms and the cause of your leg pain is uncertain.
  • A fusion is proposed for a simple disc herniation, which is usually not necessary.

  1. Which nerve is affected, and does the MRI match my symptoms?
  2. How likely is it that this settles without surgery, and over what time frame?
  3. Would an injection help, and are there risks?
  4. Would microdiscectomy or endoscopic surgery be better for me?
  5. What is the risk of the disc herniating again?
  6. What activity limits apply afterwards, and when can I return to work and fly?

Most disc herniations improve within six to twelve weeks without surgery. When surgery is needed, microdiscectomy relieves leg pain in the large majority of patients, often rapidly. Back pain may persist more than leg pain. A minority have recurrent herniation, often in the first year. Nerve weakness may take months to recover, and outcomes are best when surgery is not delayed in cases of significant weakness.

Will my disc heal without surgery?

Often yes. Most herniations shrink and symptoms improve over six to twelve weeks with activity, physiotherapy and pain relief. Surgery is for those who do not improve, or who have significant weakness.

What symptoms need emergency care?

Loss of bladder or bowel control, numbness in the saddle area or rapidly worsening leg weakness may mean cauda equina syndrome and need immediate surgery.

Can the disc herniate again?

Yes, in a small proportion of patients, most often within the first year. Staying active and good lifting technique help.

Clinical context

Working up lumbar disc herniation properly

A condition like lumbar disc herniation is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ортопедия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ортопедия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
