Laryngeal Cancer

Laryngeal cancer is a cancer of the voice box, which sits in the throat and contains the vocal cords. It is strongly linked to smoking and heavy alcohol use and mostly affects adults over 55.

О laryngeal cancer

The most common early symptom is persistent hoarseness. Others include sore throat, ear pain, difficulty or pain swallowing, a lump in the neck and, later, breathing difficulty. Diagnosis is by examination of the larynx with a flexible camera, biopsy under anaesthetic, and CT or MRI to stage it. Cancers found early on the vocal cords have a good outlook.

Early tumours are treated with radiotherapy or laser surgery, which often preserve the voice. More advanced cancers may need chemoradiotherapy to try to save the larynx, or a laryngectomy, in which the voice box is removed and a permanent breathing opening is made in the neck. Speech therapy, voice prostheses and swallowing rehabilitation follow. Treatment abroad is considered mainly for specialist head and neck surgery.

  • Biopsy pathology report from laryngoscopy, with grade and the exact site, such as glottis or supraglottis.
  • CT or MRI of the neck and larynx, plus PET-CT or chest CT to look at neck nodes and distant spread.
  • Reports from the endoscopic examination, including vocal cord movement, which affects staging.
  • Swallowing and voice assessment, and your smoking and alcohol history.
  • Dental assessment, hearing tests and kidney function, which are needed if radiotherapy or cisplatin chemotherapy is planned.

  • Laryngectomy is recommended for an early cancer, where laser surgery or radiotherapy may preserve the voice.
  • You have been advised radiotherapy alone and want to compare it with surgery for your stage.
  • The cancer has returned after radiotherapy and you want to review whether salvage surgery or other options exist.
  • You need a total laryngectomy and want to understand voice rehabilitation before you decide.

  1. What is the exact site and stage of my cancer, and are neck nodes involved?
  2. Can my voice box be kept with radiotherapy or laser surgery, and with what cure rate?
  3. If I need a laryngectomy, how will I speak and breathe afterwards?
  4. Will I need chemotherapy or radiotherapy in addition to surgery?
  5. What are the effects on swallowing and the neck, and who will help me with rehabilitation?
  6. What follow-up is needed, and what is the risk of recurrence?

Early glottic cancers have a high chance of cure, often above eighty to ninety per cent, with radiotherapy or laser surgery, usually keeping the voice. Cure rates fall as the tumour grows and spreads to neck nodes. Continued smoking worsens outcomes and raises the risk of second cancers. After laryngectomy, speech is possible through a voice prosthesis, oesophageal speech or an electrolarynx.

Will I lose my voice?

Not always. Early cancers can often be treated with radiotherapy or laser surgery that keeps the voice, though it may sound different. Total laryngectomy removes the voice box, but speech can be rehabilitated.

How do people speak after laryngectomy?

Options include a voice prosthesis through a small opening into the oesophagus, oesophageal speech or an electrolarynx. A speech therapist guides training.

Does stopping smoking matter after diagnosis?

Yes. Stopping improves response to radiotherapy, healing after surgery and survival, and reduces the risk of second cancers.

Clinical context

Working up laryngeal cancer properly

A condition like laryngeal cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
