Early on there may be few symptoms. Later, people develop tiredness, swelling, nausea, itching, poor appetite and breathlessness. It is identified by blood tests such as eGFR and creatinine, urine protein tests and ultrasound, usually after years of chronic kidney disease from diabetes, high blood pressure or other causes. Function does not usually recover once it reaches this stage.

Treatment is renal replacement. Haemodialysis or peritoneal dialysis replaces filtering, alongside diet, fluid control and medicines for anaemia, bone health and blood pressure. A kidney transplant, from a living or deceased donor, offers the best long-term outcome for suitable patients, who must take anti-rejection medicines for life. Treatment abroad is sometimes explored for transplant, subject to legal and ethical rules on donation.

  • Your underlying kidney diagnosis, such as diabetes, hypertension or glomerulonephritis, and any kidney biopsy report.
  • Recent creatinine, eGFR, potassium, haemoglobin, calcium and phosphate results, and your dialysis prescription if you are already on dialysis.
  • Blood group and tissue typing (HLA) results, and antibody screening if previously done.
  • Infection screening such as hepatitis B, hepatitis C, HIV and tuberculosis, and heart assessment including echocardiogram.
  • Details of any potential living donor, their blood group and general health, and your history of previous transplants or blood transfusions.

  • You have been told transplant is not possible but nobody has completed a full assessment of your heart, infections and other illnesses.
  • A relative wants to donate and you want an independent opinion on compatibility and donor safety.
  • You are on dialysis and have not been listed or evaluated for a pre-emptive or living-donor transplant.
  • You have had a previous failed transplant and want to understand why before trying again.

  1. What caused my kidney failure, and could the disease return in a new kidney?
  2. Am I fit for transplant, and what tests remain?
  3. What are the differences in outcomes between living and deceased donor transplants?
  4. Which anti-rejection medicines would I take, and for how long?
  5. How will my care be shared between the surgical team abroad and my kidney doctor at home?
  6. What happens if the transplanted kidney fails?

A successful transplant generally gives longer survival and better quality of life than staying on dialysis, particularly for younger, otherwise fit patients. Living-donor kidneys typically last longer than deceased-donor kidneys. Results depend on age, other illnesses such as heart disease or diabetes, matching, and taking anti-rejection medicines reliably. Regular lifelong follow-up is required, and some transplants eventually fail.

Is a transplant better than dialysis?

For suitable patients, a transplant usually offers longer survival and better quality of life. Dialysis keeps people alive but takes time and restricts diet and fluids.

How long does a transplanted kidney last?

Living-donor kidneys often function for 15 to 20 years or longer, deceased-donor kidneys somewhat less. Many factors matter, including matching and medicine adherence.

Can I travel for a transplant if I am on dialysis?

Yes, with planning. You need dialysis arranged before and after, a stable condition and infection screening, and you should stay near the transplant team for the early weeks.

Clinical context

Working up почечная недостаточность properly

A condition like почечная недостаточность is rarely well-served by jumping straight to treatment shopping. The sequence below is what a трансплантация органов specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or трансплантация органов specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
