О hodgkin lymphoma

The usual first sign is a painless lump in the neck, armpit or groin, sometimes with fevers, drenching night sweats, weight loss and itching. Diagnosis needs a lymph node biopsy, with PET-CT to stage the disease and blood tests. Most patients are diagnosed at an early stage, and the outlook is good even for advanced disease.

Treatment is chemotherapy, commonly ABVD or a similar regimen, over several cycles, sometimes with radiotherapy to affected areas, and PET scans guide the amount needed. For relapsed disease, high-dose chemotherapy with an autologous stem cell transplant is used, and immunotherapy drugs such as brentuximab and checkpoint inhibitors are options. Long-term follow-up checks for late effects on fertility, heart and thyroid.

  • Lymph node biopsy pathology confirming classical or nodular lymphocyte-predominant Hodgkin lymphoma, preferably excisional.
  • PET-CT scan reports for staging, with Ann Arbor stage and note of B symptoms (fever, night sweats, weight loss).
  • Blood tests including blood count, ESR, albumin, LDH, liver and kidney function, and HIV, hepatitis B and C status.
  • Echocardiogram and lung function tests, usually requested before chemotherapy such as ABVD or BEACOPP.
  • Chemotherapy records, interim PET results and radiotherapy details if treatment has begun.

  • Your biopsy was a needle sample only and the subtype is unclear.
  • Your PET shows incomplete response, or the disease has returned after treatment.
  • You are young and want to discuss fertility preservation before chemotherapy.
  • Radiotherapy is proposed to the chest and you want to understand the long-term heart and breast risks.

  1. What subtype and stage is my lymphoma?
  2. Which chemotherapy regimen do you recommend, and is radiotherapy needed?
  3. How will my response be checked, for instance with interim PET?
  4. How will treatment affect my fertility, heart and lungs?
  5. What happens if it comes back, and would a stem cell transplant be considered?
  6. What long-term follow-up do I need?

Hodgkin lymphoma is one of the most curable cancers. Most patients, including many with advanced disease, are cured with modern chemotherapy, and early-stage disease has particularly high cure rates. Relapsed disease can often be treated successfully with high-dose chemotherapy and autologous stem cell transplant or newer drugs. Outcome depends on stage, age and response to treatment. Survivors need long-term monitoring for late effects on the heart, lungs, fertility and second cancers.

Can I be cured?

Yes, in most cases. Hodgkin lymphoma is among the most curable cancers, including many advanced cases, and even relapses can often be cured.

Will chemotherapy make me infertile?

Some regimens can reduce fertility, especially BEACOPP. Sperm banking or egg and ovarian tissue preservation should be discussed before treatment.

How long does treatment last?

Usually about 3 to 6 months of chemotherapy, sometimes followed by radiotherapy, depending on stage and response on PET scan.

Clinical context

Working up hodgkin lymphoma properly

A condition like hodgkin lymphoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
