Gastroschisis

Gastroschisis is a birth defect in which a baby's intestines, and sometimes other organs, protrude outside the body through a small opening beside the umbilical cord. There is no protective sac covering them.

О gastroschisis

It is often seen on the mid-pregnancy ultrasound scan and raised alpha-fetoprotein may be noted. Delivery is planned at a centre with neonatal surgical care. After birth the exposed bowel is covered and kept warm, and the baby is fed intravenously. The bowel can be swollen or damaged, which may delay feeding, but most babies survive and do well.

Surgery to return the bowel to the abdomen and close the defect is needed. This can be done in a single operation or in stages using a silo, a protective pouch that lets the bowel return gradually over days. Nutrition by drip continues until the gut works. Babies with bowel atresia or damage may need further procedures. Families may travel for specialist neonatal surgery when local services are limited.

  • Antenatal ultrasound reports with dates, showing bowel appearance, bowel wall thickness, fluid volume and growth of the baby.
  • A record of the delivery plan, gestational age and birth weight, if the baby is already born.
  • Neonatal notes: how the exposed bowel was covered, whether it is twisted or damaged, and whether a silo has been placed.
  • Blood tests, nutrition plan and details of any intravenous feeding the baby is receiving.
  • Any other findings on scans, such as heart or kidney anomalies, or intestinal atresia.

  • Prenatal scans show dilated bowel, thickening of the bowel wall or poor growth, and delivery timing and place are being decided.
  • Your baby has had primary closure but feeding is not progressing after several weeks.
  • There are signs of bowel atresia, twisting or loss of bowel and long-term feeding problems are suggested.
  • You are told to transfer abroad with a newborn and the baby's stability for travel is unclear.

  1. Is my baby's gastroschisis simple, or are there signs of bowel damage or atresia?
  2. Where and when should delivery be planned?
  3. Will closure be done at once or in stages with a silo?
  4. How long might it take before feeding is established, and what is the plan for nutrition?
  5. What longer-term problems should we watch for, such as bowel obstruction or reflux?
  6. Is there any need for genetic testing or heart checks?

Most babies with simple gastroschisis survive, with survival above 90 percent in specialist centres. The main variable is bowel condition: babies with atresia, perforation or twisting (complex gastroschisis) need longer hospital stays and more procedures. Recovery of normal feeding can take weeks. Most children grow normally, though some have later bowel obstruction or reflux. Outcome depends on the amount of healthy bowel and on the quality of neonatal care.

Can gastroschisis be seen before birth?

Yes, it is usually detected on the routine mid-pregnancy ultrasound. Close monitoring and a plan for delivery near a paediatric surgical centre are then advised.

Will my baby need more than one operation?

Many babies have the bowel placed back and the wall closed in one go; others need a staged closure with a silo over days. Further procedures are needed if the bowel is damaged.

Does it happen again in later pregnancies?

Recurrence in another pregnancy is uncommon. It is usually not inherited, and its cause is not fully understood.

When can my baby feed normally?

Feeding usually starts through a drip and begins by mouth once the bowel recovers, which can take weeks.

Clinical context

Working up gastroschisis properly

A condition like gastroschisis is rarely well-served by jumping straight to treatment shopping. The sequence below is what a детская хирургия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or детская хирургия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
