Essential Tremor
Essential tremor is a common, usually inherited, condition causing rhythmic shaking of the hands, and sometimes the head or voice. It is most noticeable during movement and becomes more common with age.

The tremor appears when holding a posture or reaching, making eating, writing and pouring difficult, and may worsen with stress or caffeine. Diagnosis is clinical, separating it from Parkinson's disease, which causes tremor at rest, and thyroid or medicine-induced tremor. Blood tests may be done to exclude other causes. It usually progresses slowly over years.
Beta-blockers such as propranolol and the anti-seizure drug primidone are the main medicines, and some people benefit from topiramate or botulinum toxin injections for head or voice tremor. When tremor is disabling despite medicines, deep brain stimulation of the thalamus or focused ultrasound can reduce hand tremor. Specialist surgical centres abroad are sometimes sought for these procedures.

Essential tremor tends to progress slowly over years and does not shorten life. About half of people respond reasonably to medicines. Deep brain stimulation often reduces hand tremor substantially and reliably, though head and voice tremor respond less well, and benefit may diminish somewhat over time. Outcome depends on the severity, the response to medicine, age and general health.

Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
