Epiretinal Membrane

An epiretinal membrane is a thin layer of scar tissue on the surface of the macula, the centre of the retina. It can distort or blur central vision and mostly affects people over 50.

О epiretinal membrane

Common symptoms are blurred central vision, wavy or distorted straight lines and sometimes double vision in one eye. Diagnosis uses a retinal examination and optical coherence tomography (OCT), which shows the membrane clearly. Many membranes remain stable for years, while some slowly thicken and worsen vision.

Mild cases are observed with regular checks, as no drops or tablets remove the membrane. When distortion or reduced vision interferes with daily life, vitrectomy with membrane peel removes the scar tissue under local anaesthetic, usually as a day case. Vision improves gradually over months, though it may not return fully, and cataract can develop. Treatment abroad is a convenience choice for a routine retinal operation.

  • Optical coherence tomography (OCT) of the macula, which shows the membrane and retinal thickness.
  • Visual acuity measurements over time and a description of distortion, such as wavy lines on an Amsler grid.
  • Dilated retinal examination report to exclude other conditions, such as a macular hole or retinal tear.
  • History of previous eye surgery, retinal detachment, uveitis, laser or injections.
  • Details of cataract status, since surgery often involves removing a cataract as well.

  • Surgery has been recommended while your vision is still good and symptoms are mild, and you want to know the benefit of waiting.
  • Your vision has dropped quickly or distortion is worsening.
  • You have had an earlier peel and the membrane has returned.
  • Another eye condition such as diabetic changes or glaucoma could limit the gain you get from surgery.

  1. How thick is the membrane, and has it changed since the last scan?
  2. Is my vision loss enough to justify surgery now?
  3. What improvement can I expect in clarity and in distortion?
  4. Will I need a cataract operation at the same time?
  5. What are the risks, such as retinal tear or detachment, infection and cataract?
  6. How long is recovery, and when can I fly?

Many epiretinal membranes are stable and need no treatment. When vision is significantly affected, vitrectomy and membrane peel usually improves acuity and reduces distortion, though distortion may not disappear fully and recovery takes several months. Outcome depends on how long the membrane has been present and the state of the retina beneath. Recurrence is uncommon, and cataract often develops after surgery.

Does it always need surgery?

No. Many membranes are mild and stable and are only monitored. Surgery is considered when distortion or blurred central vision interferes with daily life.

Will surgery restore normal vision?

It usually improves vision and reduces distortion, but not always fully, and improvement can take months.

Can the membrane return?

Recurrence is uncommon after a successful peel. Cataract commonly develops faster after vitrectomy.

Clinical context

Working up epiretinal membrane properly

A condition like epiretinal membrane is rarely well-served by jumping straight to treatment shopping. The sequence below is what a офтальмология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or офтальмология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
