Edentulous Lower Jaw

An edentulous lower jaw means all the lower teeth are missing. Lower dentures often become loose because the bony ridge that supports them shrinks and the tongue pushes against them.

О edentulous lower jaw

People report loose dentures, sore gums, difficulty chewing and trouble speaking clearly. A dentist examines the ridge and takes X-rays or a cone-beam CT scan to measure bone height and locate nerves. Bone shrinkage continues for life, which is why dentures usually fit less well each year.

A conventional complete lower denture is the simplest option. A denture held by two to four implants is a well-established improvement in stability. A fixed bridge on four to six implants, such as All-on-4 or All-on-6, feels closest to natural teeth. The lower jaw is usually dense enough for implants without grafting. Treatment abroad needs planning around healing and check-ups.

  • Panoramic X-ray and CBCT scan showing the height and width of bone and the position of the nerve in the lower jaw.
  • Your existing lower denture and a description of the problems, such as movement, sore spots or difficulty chewing.
  • Medical history including bisphosphonate or bone-strengthening medicine, diabetes, smoking and head and neck radiotherapy.
  • Records of previous implants or extractions, with dates.
  • Notes on the opposing upper teeth or denture, since the bite on both sides matters.

  • You are advised to have implants placed very close to the nerve and want to know about nerve safety and alternatives.
  • You are told you have too little bone but have not been offered shorter implants or a different design.
  • You are comparing an overdENTURE on two implants with a fixed bridge on four or more.
  • You take bone-strengthening medicines and have not had their effect on jaw healing discussed.

  1. Is there enough bone in my lower jaw, and how close are the implants to the nerve?
  2. Would two implants for an overdenture be enough, or do I need a fixed bridge?
  3. What is the difference in cleaning and upkeep between the two designs?
  4. Will the implants be loaded immediately?
  5. What are the risks of implant loss, numbness and infection?
  6. How will my upper teeth be dealt with to balance the bite?

The lower jaw usually has dense bone, and implants there tend to have a high long-term success rate. An implant-retained overdenture on two implants gives a big improvement over a loose denture for most people, and a fixed bridge on four to six implants gives still better chewing. Outcome depends on bone, hygiene, smoking, diabetes and regular maintenance. Parts such as clips and acrylic wear and need replacement over time.

Why does my lower denture keep moving?

The lower ridge shrinks after tooth loss and the tongue moves the denture. Implants hold it in place far better.

How many implants do I need in the lower jaw?

Two implants can hold an overdenture, while four to six usually support a fixed bridge. The best number depends on bone and your preference.

Is the nerve at risk during lower implant surgery?

There is a small risk if bone is low, which is why a CBCT scan is used to plan implant length and position away from the nerve.

Clinical context

Working up edentulous lower jaw properly

A condition like edentulous lower jaw is rarely well-served by jumping straight to treatment shopping. The sequence below is what a стоматология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or стоматология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
