Dupuytren's Contracture
Dupuytren's contracture is a thickening of the tissue under the skin of the palm that gradually pulls one or more fingers, usually the ring and little fingers, into a bent position. It is more common in older men and runs in families.

О dupuytren's contracture
It begins as a firm lump or nodule in the palm, then forms cords that bend the finger so the hand cannot lie flat. It is usually painless. Diagnosis is clinical, with the tabletop test (the palm cannot be placed flat). Progress is unpredictable and often slow, and it cannot be reversed by exercise or splinting.
Mild disease without limits on function is simply watched. Treatment is considered once a finger bends noticeably or the tabletop test fails. Options include needle fasciotomy, collagenase injection and surgical removal of the diseased tissue (fasciectomy), with hand therapy afterwards. Recurrence is common and may need repeat treatment. People rarely need to travel abroad for this, though some seek an experienced hand surgeon.
- Photographs or measurements of finger bending at the knuckle and middle joints, and the table-top test result if recorded.
- Details of which fingers are affected on each hand and how fast the contracture has progressed.
- Records of prior treatment such as needle fasciotomy, collagenase injection or surgery, with dates.
- Medical history including diabetes, alcohol use, smoking and family history, which are linked with Dupuytren's disease.
- Notes on any similar nodules on the feet or knuckles, and whether you have a hand therapist's assessment.
- You are told to wait but the finger is bending steadily toward the palm and you want to know the best time for treatment.
- Surgery has been proposed for the middle joint and you want to understand what degree of straightening can realistically be achieved.
- The disease has returned after earlier treatment.
- Different doctors recommend different methods: needle, injection or open surgery.
- How bent is each joint, and is it the knuckle joint or middle joint that is affected?
- Which treatment is best for my fingers: needle release, enzyme injection or fasciectomy?
- How likely is the contracture to return, and how soon?
- What are the risks to the nerves and blood supply of the finger?
- What hand therapy or splinting will I need afterwards?
- Can I wait, and what would the downside of waiting be?
Dupuytren's is a chronic condition that can be straightened but not cured. Procedures can restore much of the straightening, especially at the knuckle joint, while the middle finger joint is harder to correct fully. Recurrence is common over several years, and is more likely in people with a strong family history or onset at a young age. Hand therapy and splinting after surgery help preserve the result.

Is surgery needed as soon as a finger bends?
Not always. Treatment is usually considered when the finger cannot lie flat on a table or daily use is affected, particularly when the knuckle joint bends.
Will it come back?
Recurrence is common over time, because the underlying tendency remains. It is more likely with a family history or onset at a young age.
Which treatment is best?
It depends on the fingers affected and the degree of bend. Needle release and enzyme injection are less invasive, while surgical removal can be better for severe or recurrent cases.
Clinical context
Working up dupuytren's contracture properly
A condition like dupuytren's contracture is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ортопедия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or ортопедия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
