Malocclusion (Crowding / Spacing)

Malocclusion means the teeth are crowded, spaced or do not meet properly when the jaws close. It is very common and affects children, teenagers and adults, mainly in appearance and ease of cleaning.

О malocclusion (crowding / spacing)

Signs include overlapping or gapped teeth, an overbite, underbite, crossbite or open bite, and sometimes difficulty biting or cleaning. A dentist or orthodontist diagnoses it by examining the bite and taking photographs, impressions or scans and X-rays. Without treatment crowding usually persists, and crowded teeth can make plaque control harder, though mild cases may never cause problems.

Treatment is orthodontic. Fixed metal or ceramic braces suit most cases, lingual braces sit behind the teeth, and clear aligners work well for mild to moderate problems. Some adults with severe jaw discrepancy need orthognathic surgery as well. Treatment takes many months to two years, followed by retainers, so people travelling abroad need a plan for review appointments.

  • Panoramic X-ray and, if available, a lateral cephalometric X-ray, which orthodontists usually request to assess jaw relationship and tooth position.
  • Dental photographs and impressions or digital scans of both arches, showing the bite from the front and sides.
  • A dental check-up showing no active decay or gum disease, since these are usually treated before braces are fitted.
  • A list of teeth already extracted, crowned or replaced with implants, and any previous orthodontic treatment.
  • Notes on jaw joint clicking or pain and habits such as thumb sucking or teeth grinding.

  • You have been told teeth must be extracted for treatment and are unsure whether an expansion or aligner approach could avoid it.
  • Your bite problem looks skeletal, such as a significantly under- or over-sized jaw, and surgery has or has not been mentioned.
  • You have gum recession or bone loss and have been offered braces without a gum assessment.
  • Different clinics propose very different treatment lengths or methods for the same teeth.

  1. Is my problem only in tooth position, or does the jaw contribute, and does that change the options?
  2. Do I need any teeth removed, and what happens if I choose not to?
  3. Which is better for my case: clear aligners, ceramic, metal or lingual braces, and what are the limits of each?
  4. How long will active treatment last, and how many visits are needed, given that I live abroad?
  5. What retainer will I need afterwards, for how long, and what happens if teeth drift back?
  6. Can follow-up adjustments be done by a dentist in my home country?

Orthodontic treatment generally achieves well-aligned teeth in most people, with active treatment commonly lasting from about one to two or more years depending on severity. Results depend on cooperation with aligner wear or diet and hygiene advice, the complexity of the bite, and age. Teeth tend to drift without retainers, so long-term retainer use is the main factor in keeping the result.

Do I need teeth removed for braces?

Not always. Extractions are sometimes used to relieve severe crowding, but expansion, spacing changes or small reductions between teeth can avoid it in some cases. Your orthodontist should explain why they suggest one approach.

How long does orthodontic treatment take?

Active treatment commonly lasts from about one year to two or more, depending on severity and the appliance. Retainers are then worn for years to keep teeth in place.

Are clear aligners as effective as braces?

For mild to moderate problems, aligners can work well. Complex rotations, bite corrections or jaw discrepancies may be better handled with fixed braces. Aligners also depend on wearing them most of the day.

Clinical context

Working up malocclusion (crowding / spacing) properly

A condition like malocclusion (crowding / spacing) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a стоматология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or стоматология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
