Chronic Myeloid Leukemia (CML)

Chronic myeloid leukaemia is a blood cancer caused by the BCR-ABL gene change, known as the Philadelphia chromosome. It is usually found in its slow-growing chronic phase in adults.

О chronic myeloid leukemia (cml)

Many people have no symptoms and are found through a routine blood count showing a high white cell count. Others have tiredness, night sweats, weight loss or an enlarged spleen. Diagnosis is confirmed by blood and bone marrow tests for BCR-ABL. If untreated, it can move to accelerated and blast phases.

Treatment with daily tyrosine kinase inhibitor tablets, such as imatinib, dasatinib or nilotinib, controls the disease in most people, with regular blood monitoring of BCR-ABL levels. A stem cell transplant is reserved for those whose disease resists several drugs or has progressed to an advanced phase. Treatment abroad is mainly considered for transplant, which needs a specialist centre and a matched donor.

  • Blood count with differential, and bone marrow biopsy report with cytogenetics showing the Philadelphia chromosome.
  • BCR-ABL1 quantitative PCR result on the international scale, with the date, as the baseline for monitoring.
  • Disease phase (chronic, accelerated or blast) and risk score, such as Sokal or ELTS, if calculated.
  • Medicines you are taking now, with doses and any side effects, plus heart assessment (ECG) and liver and kidney function.
  • Hepatitis B status, and cholesterol, glucose and lung or vascular history, which influence drug choice.

  • You have been diagnosed in accelerated or blast phase and need a specialist leukaemia centre's plan.
  • Your BCR-ABL level is not falling as expected, or you have stopped responding to your tablet.
  • You are considering stopping treatment or planning pregnancy and want specialist advice first.

  1. Which phase and risk score do I have, and which tyrosine kinase inhibitor is best for me?
  2. How often will BCR-ABL be tested, and what response is expected at three, six and twelve months?
  3. What are the side effects of my drug, and how will they be managed?
  4. Could I ever safely stop treatment, and under what conditions?
  5. How do I keep a reliable supply of my tablet if I travel?

With modern tyrosine kinase inhibitors, most people with chronic-phase CML reach deep responses and have a near-normal life expectancy, provided they take the tablets reliably and are monitored regularly. Some can later try treatment-free remission under specialist supervision. Outlook is worse in accelerated or blast phase, or when resistance develops, where other drugs or stem cell transplant may be needed.

Is CML curable?

Most chronic-phase patients achieve long-term control with daily tablets and live near-normal lives. True cure is uncommon, though some people can stop treatment under specialist monitoring after a sustained deep response.

Will I need a bone marrow transplant?

Most do not. Transplant is considered mainly for advanced phases, or when the leukaemia becomes resistant to several tablets.

Can I stop taking the tablet if I feel well?

You should not stop on your own. Stopping without supervision can allow the leukaemia to return quickly, and any decision to stop is made by your haematologist.

Can I travel with CML?

Yes, with enough tablets for the trip plus extra, a letter from your doctor, and a plan for monitoring tests while away.

Clinical context

Working up chronic myeloid leukemia (cml) properly

A condition like chronic myeloid leukemia (cml) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a онкология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or онкология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
