О cervical dysplasia (hsil)

It causes no symptoms and is found through cervical screening, when an abnormal smear leads to colposcopy and a biopsy. HSIL corresponds to CIN 2 or CIN 3. If left alone, a proportion of high-grade lesions progress to cancer over several years, which is why treatment is recommended. Follow-up focuses on HPV testing and repeat smears.

The abnormal tissue is removed or destroyed. The most common treatment is LEEP, large loop excision of the transformation zone, done in clinic under local anaesthetic. Cone biopsy (conisation) removes a larger cone of tissue and is used for deeper or glandular lesions. Both are very effective, and a very small risk of later preterm birth rises with larger excisions. Regular check tests continue afterwards, and HPV vaccination is advised.

  • Cervical cytology (smear) result showing HSIL, and any HPV test results with genotype.
  • Colposcopy report with biopsy results, stating CIN 2 or CIN 3 and whether the whole lesion was visible.
  • Prior screening history and any earlier cervical treatment.
  • Your pregnancy status and future pregnancy plans.
  • Details of immune conditions such as HIV or medicines that suppress immunity.

  • A hysterectomy has been advised for CIN 2 or 3, which is rarely needed.
  • The biopsy suggests cancer or glandular abnormality and the plan is unclear.
  • You are young and wish to have children and are offered an extensive excision.
  • The abnormal area extends into the canal and was not fully seen at colposcopy.

  1. Was the biopsy CIN 2 or CIN 3, and was the whole lesion seen?
  2. Do I need LEEP or cone biopsy, or could I be watched if young?
  3. What amount of tissue will be removed, and could it affect future pregnancy?
  4. What do the margins of the removed tissue show?
  5. How often will I need smears and HPV tests afterwards?
  6. Should I be vaccinated against HPV?

Treatment of high-grade cervical dysplasia is very effective, curing the great majority of women, with excision removing the abnormal tissue in a single procedure. A small proportion have abnormal cells return, especially if margins are involved or HPV persists, so follow-up with HPV testing and smears is needed. Excision may slightly raise the risk of preterm birth, particularly with deeper cone biopsies. Left untreated, some high-grade lesions progress to cancer over years.

Is HSIL cancer?

No. It is pre-cancer, and treatment is aimed at preventing cancer.

Will it affect pregnancy?

Most women conceive and deliver normally; deeper excisions slightly raise preterm birth risk.

Is LEEP painful?

It is done under local anaesthetic in minutes, with mild cramping and light bleeding afterwards.

Can it return?

Occasionally, which is why follow-up testing continues for years.

Clinical context

Working up cervical dysplasia (hsil) properly

A condition like cervical dysplasia (hsil) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a гинекология specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or гинекология specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
