О cavernoma (cerebral cavernous malformation)

Many are found by chance on MRI. When symptoms occur they include seizures, headaches, or weakness and numbness from a bleed. Some run in families and cause multiple lesions, which genetic testing can identify. MRI is the key test. The chance of bleeding is usually low each year but higher after a first bleed, and cavernomas lack the high-pressure flow of an AVM.

Cavernomas without symptoms are observed with periodic MRI. Seizures are controlled with anticonvulsants. Surgical removal is considered when a lesion has bled, causes seizures that medicines do not control, and lies in a place that can be reached safely. Stereotactic radiosurgery is sometimes used for lesions deep in the brain, though its benefit is less certain. The decision rests on location and symptoms.

  • MRI of the brain with susceptibility-weighted sequences, which show cavernomas best, with images.
  • A description of seizures, headaches or neurological symptoms, and any anti-seizure drugs used.
  • Records of previous bleeds, with dates and imaging findings.
  • Details of family history, because familial cavernomas are often multiple, and genetic test results if done.
  • EEG results if seizures have occurred.

  • Surgery is advised for a cavernoma with no symptoms and no bleeding.
  • Removal is advised for a lesion in the brainstem and you want an experienced team's view.
  • Seizures are not controlled by medicines and surgery has not been discussed.
  • Radiosurgery is proposed, which is less established for this condition.

  1. Where is my cavernoma and does it match my symptoms?
  2. Has it bled, and what is my risk of another bleed?
  3. Is surgery recommended, and what are its risks at this site?
  4. Can my seizures be controlled with medicines?
  5. Do I have more than one, and should my family be checked?
  6. What activities should I avoid?

Many cavernomas stay stable for life, and annual bleeding risk is generally low, though higher after a first bleed or in the brainstem. Surgery to remove an accessible cavernoma can stop seizures and prevent further bleeding, with a good outcome in most. Brainstem lesions carry greater surgical risk. Outcome depends on location, bleeding history and seizure type. Family forms may produce new lesions over time.

Does every cavernoma need surgery?

No. Many are simply monitored with scans and treated for seizures if needed.

Will it bleed?

Bleeding risk is usually low per year but varies with the site and history.

Is it hereditary?

Some are, especially when there are multiple lesions.

Can I fly?

Generally yes if stable; ask about seizure control first.

Clinical context

Working up cavernoma (cerebral cavernous malformation) properly

A condition like cavernoma (cerebral cavernous malformation) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a неврология и нейрохирургия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or неврология и нейрохирургия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
