Adult phimosis is a foreskin that is too tight to be pulled back over the head of the penis. It can cause pain, recurrent infection and difficulty with hygiene or sex.

Symptoms include a narrow ring of skin, ballooning of the foreskin when passing urine, soreness, recurrent balanitis and painful erections. Diagnosis is by examination, and diabetes should be checked for. A scarring form called lichen sclerosus can cause a white, hardened ring and needs separate assessment. Untreated, it may get worse.
Mild cases may respond to a course of topical steroid cream with gentle stretching over several weeks. If this fails or scarring is present, circumcision removes the tight skin and is the definitive treatment, usually as a day case under local or general anaesthetic. Preputioplasty is an alternative that keeps the foreskin. Recovery takes a couple of weeks.
