О acne scarring

Scars form when deeper inflammation damages collagen, so they often remain long after spots have cleared. They are classified by shape, and the type guides treatment. Diagnosis is a clinical examination of the skin under raking light, noting scar type, depth, and skin tone. Scars do not worsen once acne is controlled, but they rarely fade fully without treatment.

Active acne should be treated first to prevent new scars. Options then include chemical peels, microneedling with or without radiofrequency, fractional laser resurfacing, subcision for tethered scars, and fillers for some rolled scars. Most people need a series of sessions, and darker skin tones need extra care to avoid pigmentation. Treatment abroad is usually considered for access to several laser platforms.

  • Clear, well-lit photographs of the scars in different lighting, and dated photographs showing how they have developed.
  • Dermatology notes confirming that active acne is controlled and recording the scar types present.
  • History of isotretinoin use and the date it was stopped, which specialists usually consider before resurfacing procedures.
  • Details of earlier treatments such as peels, microneedling or lasers, and how your skin reacted.
  • Your tendency to keloid, hyperpigmentation or cold sores, which affects the choice of device and preparation.

  • You have darker skin and have been offered an ablative laser without discussion of post-inflammatory pigmentation risk.
  • You still have active acne and are being offered scar treatment before it is controlled.
  • Several sessions of one treatment have given little improvement and you are advised to repeat it.
  • You have thick, raised scars that may actually be keloid or hypertrophic rather than atrophic acne scars.

  1. Which types of scar do I have, and which treatment targets each one?
  2. How many sessions will I need, and what improvement is realistic rather than complete removal?
  3. What is my risk of pigment change or worsened scarring with my skin type?
  4. How long will I be red or peeling, and when can I fly home safely?
  5. Do I need subcision, fillers or punch techniques alongside energy devices?
  6. How should I protect and care for my skin during and after treatment?

Acne scars rarely disappear completely, but most people see meaningful smoothing with a combination of microneedling, radiofrequency, laser resurfacing and sometimes subcision or fillers. Improvement builds gradually over several months as collagen remodels. Deep ice-pick scars respond least. Results depend on scar type, skin tone, number of sessions and avoiding sun exposure while healing.

Can acne scars be removed completely?

Rarely. Most treatments smooth and soften scars rather than erase them, and a combination of methods usually works better than one alone.

How many sessions will I need?

Most scar treatments need a series of sessions, often three to six or more, spaced weeks apart. The number depends on scar type, depth and the method used.

Is treatment safe for darker skin?

Many options can be used, but darker skin has a higher risk of dark marks after inflammation. Treatments and settings should be chosen carefully, sometimes with skin preparation first.

Clinical context

Working up acne scarring properly

A condition like acne scarring is rarely well-served by jumping straight to treatment shopping. The sequence below is what a пластическая хирургия specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or пластическая хирургия specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
