Trigeminal neuralgia causes sudden, severe, electric shock-like pain in one side of the face. It is usually due to a blood vessel pressing on the trigeminal nerve.

Attacks last seconds and are set off by touch, chewing, talking, brushing teeth or a breeze, and can come in clusters. Diagnosis is clinical, and an MRI looks for a vessel loop or other cause, such as multiple sclerosis or a tumour. It can be extremely distressing, but it is not life-threatening.

Carbamazepine or oxcarbazepine is the first treatment and works well in most people at first, with other medicines such as gabapentin as alternatives. If medicines fail or cause side effects, procedures are offered. Microvascular decompression moves the vessel away from the nerve and gives the longest relief. Gamma Knife radiosurgery and percutaneous nerve procedures are less invasive options. Patients often travel abroad for decompression or Gamma Knife treatment from specialist centres.

  • MRI of the brain with high-resolution nerve sequences, to show a vessel pressing on the nerve and to exclude a tumour or multiple sclerosis.
  • A pain diary describing the side, trigger points, duration of attacks and pain-free periods.
  • A record of medicines tried, such as carbamazepine or oxcarbazepine, with doses, side effects and blood sodium or liver results.
  • Dental examination, because tooth problems can mimic the pain.
  • Details of any prior procedures, such as injections, balloon compression or radiosurgery.

  • Surgery has been advised but you have not tried carbamazepine or oxcarbazepine at adequate doses.
  • Medicines are causing drowsiness, rash or low sodium, or pain is breaking through.
  • Your MRI shows no vessel contact and you are unsure which procedure suits you.
  • You have constant facial pain or numbness, which may suggest another diagnosis.

  1. Is this classic trigeminal neuralgia, and does my MRI show a blood vessel on the nerve?
  2. Have I had a good trial of the right medicines?
  3. Would microvascular decompression, Gamma Knife or a needle procedure suit me best?
  4. How long does pain relief last with each option?
  5. What are the risks, such as facial numbness, hearing loss or recurrence?
  6. Is there any link to multiple sclerosis or a tumour?

Trigeminal neuralgia is not life-threatening but can be severely disabling. Most people respond to medicines at first, though many eventually need a procedure. Microvascular decompression gives the longest relief, with most patients pain-free for years, while radiosurgery and needle procedures are less invasive but pain can return and facial numbness is more common. Outcomes are better when a vessel is seen pressing on the nerve.

Is trigeminal neuralgia curable?

Procedures such as microvascular decompression can give long-lasting relief, sometimes years, but pain can return. Medicines control it for many people but do not cure it.

Do I need an MRI?

Yes. MRI helps to find a blood vessel pressing on the nerve and rules out a tumour or multiple sclerosis.

What triggers the pain?

Light touch, chewing, talking, brushing teeth, shaving or cold wind. Identifying triggers can help you cope between treatments.

Clinical context

Working up ट्राइजेमिनल न्यूरलजिया properly

A condition like ट्राइजेमिनल न्यूरलजिया is rarely well-served by jumping straight to treatment shopping. The sequence below is what a न्यूरोलॉजी और न्यूरोसर्जरी specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
