Relapses can cause visual loss, numbness, weakness, balance problems or fatigue, lasting days to weeks. Diagnosis combines the clinical history with MRI of the brain and spine, and sometimes spinal fluid tests. Without treatment, relapses can leave lasting disability, and over time some people move to a progressive phase.

Relapses are treated with a short course of high-dose steroids. Disease-modifying therapies reduce the frequency of relapses and slow accumulation of disability, ranging from injectables and tablets to infusion therapies such as ocrelizumab or natalizumab. Rehabilitation, symptom management and exercise are important. Autologous stem cell transplant is an option in selected active cases, and patients sometimes seek it abroad.

  • MRI of the brain and spinal cord with contrast, with reports and ideally earlier scans for comparison.
  • A record of every relapse, with dates, symptoms, treatment and recovery.
  • Lumbar puncture results (oligoclonal bands), evoked potential tests and eye reports if done.
  • Blood tests usually requested before treatment, including hepatitis B, varicella and TB screening, vitamin D and blood counts.
  • A list of disease-modifying treatments tried, with reasons for stopping.

  • You have new relapses or new scan lesions despite taking a disease-modifying drug.
  • Your diagnosis is uncertain, for example with atypical symptoms or few lesions.
  • A high-potency drug is advised and you want to understand the risks, including infections.
  • You are planning pregnancy or are pregnant and need to choose treatment safely.

  1. How active is my MS on scans and by relapse history?
  2. Which disease-modifying therapy suits my activity level, and what are its risks?
  3. How will we monitor whether it is working?
  4. What should I do during a relapse?
  5. How does MS treatment affect pregnancy and vaccination?
  6. What can I do about fatigue, walking and bladder symptoms?

Outcomes vary widely. Modern disease-modifying therapies substantially reduce relapses and new lesions, and many people with relapsing MS remain active for decades. Without treatment some develop gradual progression over time. Early treatment, low early disease activity and good control of risk factors such as smoking tend to give better outcomes.

Is there a cure for MS?

No cure exists yet, but disease-modifying therapies reduce relapses and new damage, and relapses are treated with steroids. Many people live active lives for decades.

Should I start treatment early?

Evidence suggests that starting disease-modifying treatment early protects the nervous system better. The choice depends on activity, safety and personal circumstances.

Can I travel with MS?

Usually yes. Heat can temporarily worsen symptoms, so plan for cooling and rest. Carry medicines, especially injectables that need refrigeration, and a letter from your doctor.

Can MS affect pregnancy?

MS does not usually reduce fertility, and relapses often decrease during pregnancy, with a higher risk after birth. Some drugs must be stopped in advance, so discuss plans with your neurologist.

Clinical context

Working up multiple sclerosis (relapsing) properly

A condition like multiple sclerosis (relapsing) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a न्यूरोलॉजी और न्यूरोसर्जरी specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
