Through a larger incision than for a standard hip replacement, the surgeon reshapes the head of the femur and covers it with a metal cap, and places a metal cup in the socket. More of your own bone is kept. It is done under spinal or general anaesthesia. Careful positioning of the parts matters a great deal for how the hip performs.

In suitable patients, particularly men with larger bones, long-term survival of the Birmingham design has been very good and high activity is often possible. Results depend on bone quality, component size and position and surgeon experience. Metal-on-metal bearings can release metal ions, so regular follow-up is advised, and it is less suited to women or people with weak bone.

Expect a stay of three to five days and walking aids at first. Plan about two weeks near your surgeon, avoid long flights for several weeks, and bring your scans. Arrange follow-up at home, including blood tests for metal levels if recommended, and keep your implant details safe.

  1. Have X-rays and sometimes CT or MRI to check femoral head bone quality and socket shape, since resurfacing needs healthy bone.
  2. Mention kidney disease, metal allergy and any previous hip surgery, and ask about blood tests for kidney function.
  3. Review blood thinners and anti-inflammatories with your doctor; do not change them yourself.
  4. Practise using crutches and prepare raised seating at home.
  5. Plan clot prevention for the flight home and check fitness to fly.
