Ureteric Stricture

A ureteric stricture is a narrowing of the ureter, the tube carrying urine from kidney to bladder. It can follow stone treatment, pelvic surgery, radiotherapy or infection and, if untreated, may damage the kidney.

Some people have flank pain or recurrent infections, while others have no symptoms until kidney function drops. Diagnosis uses ultrasound, CT urography, a kidney drainage scan and sometimes a camera examination. Strictures tend to persist or worsen, because scar tissue does not usually widen unaided, and kidney function is monitored.

A temporary stent or a nephrostomy tube can relieve blockage first. Short strictures may be dilated through an endoscope, though recurrence is common. Longer or repeated strictures are treated with reconstructive surgery, notably ureteric reimplantation into the bladder, or other repairs using a bladder flap or bowel segment. Complex cases are often referred to specialist reconstructive centres.

  • Imaging reports and discs: CT urogram, MR urogram or ultrasound showing the site, length and kidney swelling.
  • A nuclear renogram (MAG3 or DTPA) report showing the split kidney function and drainage.
  • Kidney function blood tests and recent urine culture results.
  • A history of the cause if known, such as previous stone surgery, pelvic surgery, radiation or tuberculosis.
  • Reports of any stent, nephrostomy tube or earlier endoscopic procedures, with dates.

  • You have been advised to remove a kidney, but a renogram suggests it still has useful function.
  • You have had repeated dilations or stent changes and are not told what the longer-term plan is.
  • Different surgeons disagree on whether the stricture needs open, laparoscopic or robotic repair.
  • The cause of the stricture, including the possibility of a tumour or tuberculosis, has not been clarified.

  1. Where is the stricture, how long is it, and what caused it?
  2. What share of kidney function does the affected kidney still have?
  3. Is dilation likely to work for this stricture, or should I have reconstruction?
  4. If surgery is needed, which technique do you advise and how often do you perform it?
  5. How long will a stent or tube stay in, and what are the risks?
  6. How will you check that the repair has worked, and for how long?

Outcomes depend on the cause, length and location of the stricture and on to what degree kidney function remains. Short strictures treated with reconstructive surgery have high success rates in experienced hands. Endoscopic dilation is less invasive but tends to work less well for longer or recurrent narrowings. Untreated blockage can slowly damage the kidney, so follow-up scans and kidney function tests are needed for some time after treatment.

Can a ureteric stricture damage my kidney?

Yes. A narrowing slows urine drainage and raises pressure in the kidney, which can slowly reduce its function and cause infection or stones. Scans and function tests show whether this is happening.

Is dilation enough, or is surgery needed?

Short, early strictures may respond to endoscopic dilation, but narrowing often returns. Longer or recurrent strictures usually need reconstructive surgery such as reimplantation.

Will I have a stent or tube?

Usually yes. A stent is commonly left in after repair for several weeks to let the ureter heal, and it is then removed. A nephrostomy tube may be used before surgery to drain the kidney.

Can the stricture come back?

It can, particularly after dilation. Reconstructive surgery has lower recurrence, but follow-up scans are needed to confirm that urine drains well.

Clinical context

Working up ureteric stricture properly

A condition like ureteric stricture is rarely well-served by jumping straight to treatment shopping. The sequence below is what a मूत्रविज्ञान specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or मूत्रविज्ञान specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
