transposition of great arteries (tga)

Newborns become blue within hours or days of birth, with fast breathing and poor feeding. Diagnosis is often made by antenatal scan or by echocardiography soon after birth. Survival depends on a mixing hole between the two circulations, kept open with prostaglandin infusion and, if needed, a balloon septostomy. Without treatment it is life-threatening in the first weeks.

The standard operation is the arterial switch, done in the first two weeks of life, moving the two arteries and the coronary arteries to their proper positions. Results are excellent, and most children grow up active. Lifelong cardiology follow-up checks for narrowing at the join sites and the aortic valve. Because surgery is needed urgently, it is typically performed where the baby is born, in a paediatric cardiac centre.

  • Echocardiogram reports describing the arterial connection, any hole between the chambers or ventricular septal defect, and the coronary artery pattern.
  • Oxygen saturations and details of any balloon atrial septostomy or prostaglandin treatment in the first days.
  • Cardiac CT, MRI or catheter study, if requested, showing the coronary arteries before surgery.
  • Birth weight, gestational age and growth records.
  • Previous operations and a summary of medicines, including diuretics.

  • Surgery has been delayed beyond the early newborn weeks, and your child's oxygen levels are unstable.
  • The coronary artery pattern is unusual, and you want a surgeon with a track record in complex arterial switch.
  • Your older child or adult was treated with an atrial switch (Mustard or Senning) and now has rhythm or heart function problems.
  • A ventricular septal defect or pulmonary obstruction complicates the surgery.

  1. What exactly is the anatomy, including the coronary arteries?
  2. When should the arterial switch be done, and what happens until then?
  3. What is your centre's survival rate for this operation?
  4. Which complications, such as coronary narrowing or leaking valves, can occur later?
  5. What follow-up tests will my child need as they grow?
  6. Can a child with this repair play sport and have a normal life?

The outlook after an arterial switch operation is very good in experienced centres, with the large majority of babies surviving and growing into healthy adults. Long-term issues can include narrowing of the pulmonary arteries, leakage of the aortic valve and, rarely, problems with the coronary arteries, so lifelong cardiology follow-up is advised. Outcomes depend on the coronary anatomy, associated defects and the timing and quality of the surgery.

    transposition of great arteries (tga)

सामान्य प्रश्न transposition of great arteries (tga)

Can a baby survive without treatment?

Not for long. Without a connection allowing blood to mix, oxygen levels stay dangerously low, so newborns need urgent specialist care and usually an arterial switch operation in the first weeks.

Will my child lead a normal life?

Most children do well after arterial switch surgery, attending school and being active, though lifelong cardiology follow-up is needed to detect late problems.

Can adults treated with an atrial switch have problems?

Yes. Older Mustard or Senning repairs can lead to rhythm problems or weakening of the right ventricle, so regular specialist review is important.

Clinical context

Working up transposition of great arteries (tga) properly

A condition like transposition of great arteries (tga) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a हृदय शल्य चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
