Leaks happen when pressure rises inside the abdomen, without a sudden urge to pass urine. Assessment includes a history, pelvic examination, a bladder diary and urine test, and sometimes urodynamic tests to confirm the type of incontinence. It is not dangerous, but can limit exercise, work and social life, and it often improves with treatment.

First-line care is pelvic floor muscle training under a physiotherapist, weight loss, reducing caffeine and treating a cough. Vaginal devices can help, and duloxetine is occasionally used. If symptoms persist, surgery is offered: a mid-urethral sling is common, with bulking injections or colposuspension as alternatives. People travelling for incontinence surgery usually do so for specialist urogynaecology care after conservative treatment has not worked.

  • A bladder diary over several days recording fluid intake, leaks and triggers.
  • Urine test results to rule out infection, and a record of any previous urinary tract infections.
  • Urodynamic study report, if done, showing whether the leakage is purely stress or mixed with urge incontinence.
  • Details of pelvic floor physiotherapy tried, how long and with what supervision.
  • A history of births, previous pelvic or incontinence surgery, any mesh used, and prolapse symptoms.

  • Surgery has been offered but you have not had a supervised course of pelvic floor exercises.
  • Your symptoms include strong urgency as well as leaking, because mixed incontinence responds differently.
  • You have had a previous sling or mesh procedure and symptoms are returning or new pain has appeared.
  • You are unsure which sling or technique is being proposed and what its long-term track record is.

  1. Is my leakage purely stress type, or mixed with urgency?
  2. To what extent could supervised pelvic floor training help me before I consider surgery?
  3. Which procedure do you recommend, and what is its long-term success record?
  4. What are the risks, such as difficulty emptying the bladder, pain or mesh complications?
  5. Do I have a prolapse that should be dealt with at the same time?
  6. If I plan future pregnancies, how would that affect the timing of surgery?

Pelvic floor training cures or improves many women with mild to moderate stress incontinence, if done correctly for several months. When surgery is needed, a mid-urethral sling cures or greatly improves leakage in most women, with good results lasting for many years. Success is lower with mixed symptoms, previous failed surgery or severe sphincter weakness. Weight loss and avoiding smoking-related cough help results.

Can stress incontinence be cured without surgery?

Often it can be greatly improved. Supervised pelvic floor exercises, weight loss and reducing bladder irritants help many women, especially with mild or moderate leakage. Surgery is usually offered if these fail.

Will the leakage come back after a sling?

Most women stay dry or much improved for many years, but some leakage can return, especially with weight gain, chronic cough or later childbirth. Your surgeon should explain success rates for the specific procedure.

Should I wait until I have finished having children?

Often yes. Pregnancy and vaginal delivery can undo the benefit of surgery, so many surgeons advise completing your family first and starting pelvic floor therapy in the meantime.

Clinical context

Working up stress urinary incontinence properly

A condition like stress urinary incontinence is rarely well-served by jumping straight to treatment shopping. The sequence below is what a मूत्रविज्ञान specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or मूत्रविज्ञान specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
