Typical symptoms are crushing chest pain spreading to the arm or jaw, sweating, nausea and breathlessness. The diagnosis is made quickly from the ECG, which shows ST elevation, supported by a blood test for troponin. Every minute counts, and calling emergency services immediately matters. Complications include heart failure, rhythm disturbances and, without prompt treatment, death.
The standard treatment is primary angioplasty, in which the blocked artery is opened with a balloon and a drug-eluting stent, ideally within about 90 minutes of reaching hospital. Where this is not available quickly, clot-dissolving drugs are given first. Aspirin and other antiplatelet drugs, statins, beta-blockers and cardiac rehabilitation follow. Because of the urgency, treatment abroad does not apply to the heart attack itself, only to later planned care.
- This is an emergency: call emergency services immediately, because the benefit of treatment falls with every hour, and travelling abroad is not an option at this stage.
- After stabilisation, keep the ECG tracings, troponin results and angiogram images from the heart attack.
- Keep the discharge summary showing which artery was treated, any stent type and the heart's pumping strength.
- A full list of heart medicines, including blood thinners, with the dates they started.
- Echocardiogram reports from before and after the event, plus cholesterol, HbA1c and kidney results.
- Your heart pumping strength remains low after the attack and you want advice on heart failure treatment or a defibrillator.
- The angiogram showed blockages in other arteries and you are unsure whether to stent them or consider bypass.
- You have chest pain, breathlessness or palpitations after discharge, which need urgent assessment.
- You were told your risk is unclear after a normal angiogram, and the cause of the heart attack has not been explained.
- Which artery was blocked, and what extent of heart muscle was damaged?
- Do I have other narrowings that still need treatment?
- How strong is my heart now, and could that improve over the coming months?
- Which medicines must I take, and for how long, especially blood thinners after a stent?
- When is it safe to return to work, travel by air and be physically active?
- What warning symptoms mean I should seek emergency help again?
Survival after a STEMI has improved greatly when the artery is opened quickly, and most people who reach hospital in time survive. The extent of lasting damage depends on how long the artery was blocked and which vessel was involved. Many people return to normal activities, but heart weakness can develop in some. Taking medicines faithfully, cardiac rehabilitation and stopping smoking strongly reduce the chance of another heart attack.

What should I do if I suspect a heart attack?
Call emergency services immediately and do not drive yourself. Chewing aspirin may be advised by the dispatcher if you are not allergic. Treatment to reopen the artery works best in the first hours after symptoms begin.
Will I need a stent after a STEMI?
Most people have a stent placed during emergency angioplasty to keep the artery open. You will then need blood-thinning medicines for a period, and other medicines and lifestyle changes to prevent another heart attack.
When can I fly or travel after a heart attack?
Your cardiologist decides based on your recovery, heart function and any ongoing symptoms. Flying is often allowed after a week or two in uncomplicated cases, but get written clearance first and carry your medicines and records.
Clinical context
Working up acute myocardial infarction (stemi) properly
A condition like acute myocardial infarction (stemi) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a हृदय शल्य चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate urgently
- Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
- Loss of consciousness, severe persistent vomiting, or any focal neurological event.
- Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
- If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
