Many tumours cause no symptoms and are found incidentally on ultrasound or CT. When symptoms occur they include blood in the urine, a flank lump, persistent flank pain, weight loss or fever. Diagnosis uses contrast CT or MRI to assess size and spread, and biopsy is used selectively. Treatment depends on stage, as small tumours are usually slow-growing.

Localised tumours are treated surgically. Partial nephrectomy, often robotic, removes the tumour and preserves kidney function, while radical nephrectomy removes the whole kidney when the tumour is large or central. Small tumours in frail patients may be ablated or watched. Advanced disease is treated with immunotherapy and targeted drugs. Patients travel for robotic surgery or specialist oncology.

  • A contrast-enhanced CT or MRI of the abdomen showing the tumour size, position and any involvement of the renal vein or caval vein.
  • A chest CT, to check for spread to the lungs, which is routine in staging.
  • Blood tests including creatinine and eGFR, haemoglobin and calcium, plus a split renal function scan if kidney function is in question.
  • Biopsy results if one was done, showing the subtype such as clear cell or papillary and the grade.
  • Details of other health problems, especially hypertension, diabetes and any family history of kidney cancer or genetic syndrome.

  • You have been advised to have the whole kidney removed for a small tumour, and partial nephrectomy has not been discussed.
  • Your tumour is large, involves the renal vein or caval vein, or appears to have spread, and surgery planning is complex.
  • You have only one kidney, reduced kidney function or tumours in both kidneys.
  • You are older or unwell and have been told you must have surgery, but surveillance or ablation might be reasonable for a small tumour.

  1. What are the stage and grade of my tumour, and what subtype is it?
  2. Can my tumour be removed with a partial nephrectomy, and what is the effect on my remaining kidney function?
  3. Is a robotic or laparoscopic approach suitable, and how experienced is the surgeon with it?
  4. Are surveillance, ablation or radiotherapy reasonable alternatives for me?
  5. What follow-up scans will I need and for how long, and what signs of recurrence should I look for?
  6. If the cancer has spread, which drug treatments could be used?

Outlook depends largely on stage. Small tumours confined to the kidney are often cured by surgery, and five-year survival is high. Larger tumours, spread to veins or lymph nodes, and distant spread lower the outlook, though modern immunotherapy and targeted drugs have improved control of advanced disease. Follow-up scans are needed for years because recurrence can occur late.

Do I always need the whole kidney removed?

No. Small tumours can often be removed with partial nephrectomy, which preserves kidney function. Larger or centrally placed tumours may need full removal.

Can I live a normal life with one kidney?

Yes, most people do. The remaining kidney grows to compensate, though kidney function is lower, so blood pressure and kidney tests are followed up.

Can kidney cancer come back?

Yes, sometimes years later, in the kidney area or in distant organs such as the lungs. Regular follow-up scans are used for this reason, and the risk depends on stage and grade.

Is a biopsy necessary?

Not always. Many masses are treated based on scans alone, but biopsy can help when surveillance, ablation or drug treatment is being considered.

Clinical context

Working up kidney (renal cell) cancer properly

A condition like kidney (renal cell) cancer is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ऑन्कोलॉजी (कैंसर) specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ऑन्कोलॉजी (कैंसर) specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
