Symptoms are nausea, vomiting of undigested food, early fullness, bloating and upper abdominal pain, which can lead to weight loss and malnutrition. Diagnosis needs an endoscopy or imaging to exclude obstruction, then a gastric emptying study, usually a scan after a test meal. Blood sugar control is important in diabetes, and symptoms often fluctuate.
Treatment begins with small, low-fat, low-fibre meals, stopping medicines that slow the stomach, tight glucose control and anti-sickness or prokinetic drugs. For severe cases there are botulinum toxin injection into the pylorus, gastric electrical stimulation, feeding tube support and pyloric procedures such as peroral endoscopic myotomy (G-POEM). Patients travel for these procedures when local expertise is limited.
- A gastric emptying study, usually a four-hour scintigraphy scan, with the report showing percentage retained at set times.
- Upper gastrointestinal endoscopy report to exclude blockage, ulcers or other causes of vomiting.
- A list of medicines tried, such as metoclopramide, domperidone or erythromycin, with doses, side effects and responses, plus any opioids or GLP-1 medicines you take.
- HbA1c and recent blood results including electrolytes, albumin and weight trend, because malnutrition affects surgical risk.
- Records of any feeding tube, gastric stimulator or previous stomach or vagus nerve surgery.
- Your gastric emptying test was done while you were taking opioids or other drugs that slow the stomach, so the result may be unreliable.
- You have been advised to have a pyloric procedure or a stimulator but have not had a full review of causes and alternatives.
- You are losing weight, being admitted for dehydration or relying on tube feeding despite treatment.
- Your symptoms are mostly pain, which responds less well to gastroparesis procedures and needs another explanation.
- Is my gastroparesis proven by a properly done emptying study, and are there other explanations for my vomiting?
- Which diet and drug options have I not yet tried, and could my diabetes control be improved?
- Would I be a suitable candidate for pyloric therapy such as G-POEM, and what is the chance it relieves my main symptoms?
- What are the risks of the procedure, including leaks, bleeding or reflux, and how would they be managed?
- Do I need a nutrition plan or a feeding tube first to be safe for treatment?
- How will we judge success and what follows if symptoms continue?
Gastroparesis is usually a long-term condition, and many people stabilise with diet changes, medicines and good diabetes control. Refractory cases need more intensive care. Pyloric procedures such as G-POEM help a proportion of carefully selected patients, particularly those with delayed emptying confirmed on testing, but results vary and are not guaranteed.

Is gastroparesis permanent?
It is often long-term, particularly when linked to diabetes, but symptoms fluctuate and can improve with diet changes, good glucose control and medicines. Post-viral gastroparesis sometimes improves over time.
Can a procedure cure gastroparesis?
Procedures such as pyloric myotomy can reduce symptoms in selected patients, but they do not repair the underlying nerve or muscle problem. Results are best when emptying is clearly delayed on testing.
What should I eat with gastroparesis?
Small, frequent meals that are low in fat and fibre are usually advised, with liquids or blended foods during flares. A dietitian can tailor this, especially if you are losing weight.
Clinical context
Working up gastroparesis (refractory) properly
A condition like gastroparesis (refractory) is rarely well-served by jumping straight to treatment shopping. The sequence below is what a जीआई सर्जरी specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.
Diagnostic workup, in sequence
- First-line clinical assessment
History, physical exam, and risk-factor review by a senior generalist or जीआई सर्जरी specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.
- Standard imaging + labs
Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.
- Specialist diagnostic workup
Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.
- Multidisciplinary review (where indicated)
Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.
When to escalate to specialist review
- New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
- Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
- Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
- A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.
Before you book international travel
- Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
- Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
- Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
- Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
- Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
