Pediatric Hypospadias

Hypospadias is a difference in boys in which the urinary opening sits on the underside of the penis instead of the tip. It is present from birth and varies from mild to severe.

Boys may pass urine in a downward or split stream, and the foreskin looks incomplete, with the penis sometimes curving downward. The diagnosis is made by examination at birth. Circumcision should be avoided before assessment because the foreskin may be used in repair. Severe forms need checks for other conditions.

Surgical repair is the main treatment and is usually advised between about six and eighteen months of age, to reshape the urethra, straighten the penis and give a normal appearance. Mild distal forms often need one operation, while severe ones may need staged repairs. Complications such as fistula or narrowing need follow-up. Treatment abroad is considered for specialist paediatric urology teams.

  • Detailed examination notes and photographs showing the position of the opening (distal, mid-shaft or proximal), curvature, and foreskin and scrotal features.
  • Records of urine stream, any infections, and whether both testicles are in the scrotum.
  • For severe cases, hormone and genetic tests or pelvic ultrasound, which are usually requested to assess differences of sex development.
  • Any previous surgery, with operation notes and complications such as fistulas or narrowing.
  • Instruction that the child must not be circumcised before the repair, as foreskin tissue may be needed.

  • Circumcision has been suggested before hypospadias has been properly assessed.
  • Both testicles are not felt in the scrotum with a severe hypospadias and no one has considered further tests.
  • Previous repair has failed with a fistula, narrowing, or persistent curvature.
  • The surgeon has little regular experience of hypospadias repair, particularly for proximal types.

  1. How severe is my son's hypospadias and does he have curvature?
  2. When is the best age for repair and how many stages will be needed?
  3. What technique will be used and how do you use the foreskin?
  4. What are the chances of fistula, narrowing or needing further surgery?
  5. How long will the dressing or catheter stay?
  6. What follow-up is needed through puberty?

Outcomes are good, especially for distal hypospadias, where one operation usually gives a straight penis and a stream directed forwards. Proximal types more often need staged repair and have higher rates of complications such as fistula, narrowing or curvature returning, sometimes requiring revisions. Most boys urinate standing and have normal function later in life. Results depend on severity, the surgeon's experience, tissue quality and healing.

Is surgery necessary?

Surgery is usually advised for function and appearance. Very mild forms may be left alone after specialist advice.

What age is best?

Often between six and eighteen months, as it is generally well tolerated and psychologically gentler.

Will my son need more than one operation?

Distal types often need one, while proximal ones often need two stages or later revisions.

Will it affect fertility?

Mild forms usually do not. Severe forms may be associated with other conditions, which your team will assess.

Clinical context

Working up pediatric hypospadias properly

A condition like pediatric hypospadias is rarely well-served by jumping straight to treatment shopping. The sequence below is what a बाल शल्य चिकित्सा specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate to specialist review

  • New, persistent, or worsening symptoms beyond the usual pattern — particularly anything that disrupts work, sleep, or daily function for >2 weeks.
  • Symptoms accompanied by red-flag features (unintentional weight loss, night sweats, bleeding, severe pain, neurological signs).
  • Any "this feels different" instinct — especially with recurrent or chronic conditions where you know your baseline well.
  • A second-opinion misalignment with your current care plan — when two qualified clinicians disagree, the case warrants a third review before committing.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
