head and neck squamous cell carcinoma

It may present as a persistent mouth ulcer, sore throat, hoarse voice, painful swallowing or a firm lump in the neck from spread to lymph nodes. Diagnosis needs endoscopic examination and biopsy, with CT, MRI or PET-CT for staging, and testing for HPV in throat tumours. Spread to neck nodes is common and affects prognosis.

Early tumours are treated with surgery or radiotherapy. When lymph nodes are involved, neck dissection removes them, and radiotherapy such as intensity-modulated radiotherapy (IMRT) is given after surgery or together with chemotherapy as a non-surgical option. IMRT shapes the dose to spare salivary glands and nearby structures. Swallowing, speech and dental care are essential, and some patients travel abroad for specialist surgery or radiotherapy.

  • Biopsy report confirming squamous cell carcinoma, with grade, depth of invasion and p16 or HPV testing for oropharyngeal tumours.
  • CT or MRI of the neck and PET-CT reports describing the tumour and any involved lymph nodes.
  • Panendoscopy findings, and a dental and swallowing assessment before radiotherapy.
  • Neck dissection pathology showing the number of nodes involved and extranodal spread, if surgery was done.
  • Radiotherapy plans, chemotherapy records and recent blood and kidney function results.

  • The neck nodes are involved and the extent of neck dissection or radiotherapy field is unclear.
  • Pathology shows close margins or extranodal spread, and you are unsure whether further treatment is needed.
  • Your treating team recommends radical surgery and you want an organ-preserving option compared.
  • The cancer has recurred or a second tumour has appeared.

  1. What are the tumour depth, grade and lymph node findings?
  2. Do I need a neck dissection, and which levels will be removed?
  3. Will I need radiotherapy or chemoradiotherapy afterwards, and why?
  4. Is the cancer HPV-related, and how does that change my outlook?
  5. What effects on speech, swallowing and shoulder movement should I expect?
  6. How often will I be reviewed, and what symptoms should prompt early contact?

Prognosis depends on the stage, site, nodal involvement and HPV status. Early disease is often cured, while nodal spread lowers survival, and extranodal spread or positive margins increase the chance of recurrence, which is why post-operative radiotherapy is used. HPV-positive oropharyngeal cancers have a better outlook. Continued smoking and drinking worsen results and raise the risk of second cancers. Most recurrences appear within the first two years, so regular review matters.

    head and neck squamous cell carcinoma

सामान्य प्रश्न head and neck squamous cell carcinoma

Do I need my neck lymph nodes removed?

Often if cancer has spread to the nodes or has a high chance of doing so. Selective neck dissection removes the at-risk levels, and radiotherapy is an alternative in some cases.

Why is radiotherapy given after surgery?

When pathology shows features such as close margins or extranodal spread, radiotherapy, sometimes with chemotherapy, lowers the chance of recurrence.

Can the cancer come back?

Yes, most commonly in the first two years, which is why close follow-up is needed. Stopping smoking and alcohol reduces recurrence and second cancers.

Clinical context

Working up head and neck squamous cell carcinoma properly

A condition like head and neck squamous cell carcinoma is rarely well-served by jumping straight to treatment shopping. The sequence below is what a ऑन्कोलॉजी (कैंसर) specialist would expect to see completed — and the absence of any step is the right cue to delay travel decisions.

Diagnostic workup, in sequence

  1. First-line clinical assessment

History, physical exam, and risk-factor review by a senior generalist or ऑन्कोलॉजी (कैंसर) specialist. Symptoms list, family history, prior workup notes, and current medication list should travel with the patient.

  2. Standard imaging + labs

Baseline blood work, condition-specific imaging (ultrasound / X-ray / MRI / CT — depending on indication), and ECG / pulmonary function as relevant. Bring digital copies of any prior imaging — saves repeat scans and the cost that goes with them.

  3. Specialist diagnostic workup

Procedure-specific tests — biopsy, cardiac catheterisation, endoscopy, or genetic / molecular profiling depending on the suspected diagnosis. Done at the centre that will treat, ideally, so results integrate cleanly into the treatment plan.

  4. Multidisciplinary review (where indicated)

Cancer, complex cardiac, transplant, and rare-disease cases should be reviewed by a multidisciplinary panel — surgical, medical, imaging, and pathology — before any treatment commits. Single-discipline plans for complex disease are a quality red flag.

When to escalate urgently

  • Acute worsening of established symptoms — sudden chest pain, shortness of breath at rest, neurological deficit, severe bleeding, persistent high fever.
  • Loss of consciousness, severe persistent vomiting, or any focal neurological event.
  • Symptoms in the early hours of onset that fit a time-sensitive treatment window (stroke, MI, sepsis) — every hour costs outcome.
  • If you are travelling internationally for treatment and develop these symptoms in transit, divert to the nearest emergency department; do not wait to reach the planned destination.

Before you book international travel

  • Confirm your specific diagnosis is fully worked up before you commit to a treatment plan abroad. Travelling for the workup is fine; travelling for treatment without the workup done is not.
  • Get your case reviewed remotely by the destination centre's team before you book flights — credible international patient programs review imaging + reports before they confirm the surgical plan.
  • Bring a digital + physical copy of your full medical record: imaging on USB drive in DICOM format, labs in PDF, current medication list with dosages, allergies, and any prior surgical reports.
  • Plan for the post-op rehabilitation phase before you fly — the surgery itself is rarely the bottleneck.
  • Confirm that follow-up imaging and any necessary in-person reviews can be done at home — a treatment plan that requires repeat international trips is more burdensome (and more expensive) than the headline procedure.
